经阴道超声联合直肠超声检查对深部浸润性子宫内膜异位症患者的诊断分析

目的 分析经阴道超声联合直肠超声检查对诊断深部浸润性子宫内膜异位症患者的临床价值.方法 选取2014年1月至2018年3月84例经手术病理诊断为深部浸润性子宫内膜异位症患者,术前分别进行阴道超声与直肠超声检查,单一应用阴道超声为对照组,阴道超声联合直肠超声为观察组,比较两组诊断结果与手术病理结果的检出阳性率.结果 对照组检查84例患者检出阳性者69例,其中单纯型27例,穹窿型14例,直肠型28例,漏诊15例,漏诊率为17.86%;观察组检查84例患者检出阳性者80例,其中单纯型29例,穹窿型20例,直肠型31例,漏诊4例,漏诊率为4.76%.观察组检出阳性率为95.24%显著高于对照组82.14%,差异有统计学意义(P<0.05);观察组对骶骨韧带、阴道壁、阴道直肠隔以及直肠诊断准确性显著高于对照组,差异有统计学意义(P<0.05).结论 经阴道超声联合直肠超声检查诊断深部浸润性子宫内膜异位症患者具有显著的临床应用价值,并可进行有效分型,为临床治疗提供可靠参考依据,可作为有效诊断方式.

Objective To analyze the clinical value of transvaginal ultrasound combined with transrectal ultrasonography in the diagnosis of patients with deep invasive endometriosis.Methods 84 patients with deep invasive endometriosis diagnosed by surgery and pathology in our hospital from January 2014 to March 2018 were selected as the research subjects,transvaginal ultrasound and transrectal ultrasonography were performed before operation,single application of vagina ultrasound was selected as the control group,transvaginal ultrasound combined with rectal ultrasound was selected as the observation group,the positive rates of the two groups were compared between the two groups.Results In the 84 patients of control group,69 cases were found positive,including 27 cases of simple type,14 cases of tornix type,28 cases of rectum type,15 cases were missed diagnosis,and the rate of missed diagnosis was 17.86%.In the 84 patients of observation group,80 cases were found positive,including 29 cases of simple type,20 cases of fornix type,31 cases of rectum type,4 cases were missed diagnosis,and the rate of missed diagnosis was 4.76%.The positive rate of the observation group was 95.24%(80/84),which was significantly higher than that of the control group 82.14%(69/84),and the difference was statistically significant(P<0.05);the diagnostic accuracies of the sacral ligament,vaginal wall,rectovaginal septum and rectum in the observation group were significantly higher than those in the control group(P<0.05).Conclusion Transvaginal ultrasound combined with transrectal ultrasonography is of great clinical value in the diagnosis of deep invasive endometriosis,it can be used for effective typing and provide reliable reference for clinical treatment,it can be used as an effective diagnostic method.
