针刺联合神经肌肉电刺激治疗脑卒中后吞咽障碍疗效探讨

Acupuncture combined with Neuromuscular Electrical Stimulation in Treatment of Dysphagia after Stroke

[目的]探讨针刺联合神经肌肉电刺激对脑卒中后吞咽障碍患者病情严重程度及并发症的影响。[方法]选取脑卒中后吞咽障碍患者100例,以随机数字表法分为对照组和观察组各50例,分别采用神经肌肉电刺激单用和在此基础上加用穴位针刺辅助治疗;比较两组患者近期疗效,治疗前后临床症状评分、日本藤岛一郎吞咽困难分级量表评及并发症发生率。[结果]观察组患者近期治疗总有效率显著高于对照组(P<0.05);观察组患者治疗后临床症状评分和日本藤岛一郎吞咽困难分级量表评分均显著优于对照组、治疗前(P<0.05)。[结论]穴位针刺联合神经肌肉电刺激治疗脑卒中后吞咽障碍可有效缓解相关症状体征,促进吞咽功能恢复,并有助于降低相关并发症风险。

[Objective]To explore the effect of acupuncture combined with neuromuscular electrical stimulation on the severity and complications of dysphagia after stroke. [Method]A total of 100 patients with dysphagia after stroke were selected and randomly divided into control group and observation group with 50 cases in each group. Neuromuscular electrical stimulation was used alone and acupoint acupuncture was added on this basis. The short-term curative effect of the two groups was compared,including clinical symptom score before and after treatment,dysphagia grading scale of Ichiro Fujima and the incidence of complications. [Result]The total effective rate of the observation group was significantly higher than that of the control group(P <0.05). The clinical symptoms and dysphagia rating scale scores of the observation group after treatment were significantly better than those of the control group and before treatment( P<0.05).[Conclusion]Acupuncture at acupoints combined with neuromuscular electrical stimulation can effectively relieve the symptoms and signs of dysphagia after stroke,promote the recovery of swallowing function and help to reduce the risk of related complications.
