丘脑底核脑深部电刺激术治疗帕金森病患者构音障碍的早期效果 被引量:4

Early efficacy of deep brain stimulation on subthalamic nucleus in the treatment of dysarthria in patients with Parkinson's disease

摘要 目的探讨丘脑底核(STN)脑深部电刺激术(DBS)对帕金森病(PD)患者的早期疗效.方法选取2016年11月至2018年11月寿光市人民医院收治的76例PD患者,所有患者均使用多巴胺能药物治疗后效果不理想,或无法耐受药物不良反应,或出现药物难以控制性震颤.所有患者均进行STN-DBS治疗,观察其疗效及不良反应发生情况.结果术后6个月,患者非运动症状评估量表评分、帕金森病评分量表评分均低于术前,差异有统计学意义(P<0.05);简易智能状态检查量表评分、言语流畅性评分与术前比较差异未见统计学意义(P>0.05).术后,76例患者中出现一过性头痛1例,无症状性颅内积气3例,眼睑青肿1例,短暂性意识模糊1例,所有不良反应均较轻微,未影响治疗.结论STN-DBS治疗可有效改善PD患者帕金森症状及非运动症状,且不会引起认知损伤及构音障碍,早期疗效显著. Objective To observe the early efficacy of deep brain stimulation(DBS)on subthalamic nucleus(STN)in the treatment of patients with Parkinson's disease(PD).Methods Seventy-six patients with PD treated in Shouguang People's Hospital from November 2016 to November 2018 were selected as research objects.All the patients were treated with dopaminergic drugs,but didn xt have satisfactory efficacy,or could not tolerate adverse drug reactions,or suffered from uncontrollable tremor.All patients were treated by STN-DBS,and the efficacy and incidence of adverse reactions were observed.Results After treatment,the non-motor symptom scale and unified Parkinson's disease rating scale scores decreased significantly(P<0.05);however,mini-mental state examination score and speech fluency score did not change significantly 6 months after operation(P>0.05).There was 1 case transient headache,3 cases asymptomatic intracranial effusion,1 case blepharoma and 1 case transient blurry consciousness.All the adverse reactions were slightly unaffected.Conclusions STN-DBS can effectively improve Parkinson's and non-motor symptoms of patients with PD,without causing cognitive impairment and dysarthria.And it has great early effects.
