摘要
目的探究木丹颗粒联合甲钴胺对2型糖尿病周围神经病变气虚络阻证的疗效及对神经电生理和氧化应激指标的影响,为2型糖尿病周围神经病变的诊疗提供临床指导。方法前瞻性选择自2016年1月至2018年3月衡水市第五人民医院收治的160例2型糖尿病周围神经病变(气虚络阻证)患者作为研究对象。按随机数表法,将患者随机分成观察组100例和对照组60例。对照组采用甲钴胺结合常规降糖治疗,观察组在对照组基础上加用木丹颗粒,疗程4周。比较两组治疗前后的多伦多临床评分系统(TCSS)和患者下肢神经的传导速度,血清超氧化物歧化酶(SOD)和丙二醛(MDA)等氧化应激指标,观察两组肌电图疗效。结果治疗前,两组患者TCSS评分、运动神经传导速度和血清SOD、MDA水平比较,差异无统计学意义(P>0.05);治疗后,两组患者上述各指标均显著改善(P<0.05)。与对照组相比,观察组患者TCSS评分显著降低,SOD水平显著上升,MDA水平显著降低,两组比较差异有统计学意义(P<0.05)。治疗后,观察组患者下肢神经传导速度(MCV、SCV)和肌电图疗效明显优于对照组,两组比较差异有统计学意义(P<0.05)。结论木丹颗粒联合甲钴胺治疗2型糖尿病周围神经病变气虚络阻证效果显著,可明显改善患者临床症状,提高神经传导速度和抗氧化应激能力,值得临床推广。
Objective To explore the Therapeutic effect of Mudan granule combined with Mecobalamin on DPN(syndrome of deficiency of Qi and collaterals)and its influence on neuroelectrophysiology and oxidative stress index,so as to provide clinical guidance for the diagnosis and treatment of DPN.Methods 160 patients with DPN(Qi deficiency and collateral obstruction syndrome)admitted to our hospital were selected as the study subjects from January 2016 to March 2018.According to the random number table method,the patients were randomly divided into the observation group 100 cases and the control group 60 cases.The TCSS and NCV of lower extremity,serum SOD and MDA were compared before and after treatment,and the effects of EMG were observed.Results Before treatment,there was no significant difference in TCSS,NCV of lower extremity,serum SOD and MDA levels between the two groups(P>0.05).After treatment,the above indexes were significantly improved(P<0.05).Compared with the control group,SOD in the observation group were significantly higher,TCSS and MDA was significantly lower(P<0.05).After treatment,the MCV,SCV and electromyography in the observation group were significantly better than those in the control group(P<0.05).Conclusion Mudan granule combined with Mecobalamin is effective in treating DPN(syndrome of deficiency of Qi and collaterals).Both of them can significantly improve the clinical symptoms,nerve conduction velocity and antioxidant stress ability of patients,which were worthy of clinical promotion.
