PDCA循环在手术室医院感染质量管理中的应用研究

Application of PDCA circulation in quality management of nosocomial infection in operating room

目的探讨PDCA循环在控制手术室院感中的作用。方法收集2015-2018年荆州市中医医院手术室实施PDCA循环前后的临床资料,按其管理方法分为常规组和PDCA组。回顾性分析两组在手术室院感质量管理方面的成效。结果PDCA组在环境、消毒隔离、安全及物品药品等管理评分中均高于常规组(P<0.01);手术切口病原菌检出率低于常规组(P<0.01);手术切口感染率低于常规组(P<0.01);PDCA组Ⅰ类、Ⅱ类、Ⅲ类手术切口感染率分别为1.14%、3.23%和7.02%,均明显下降,组间比较差异有统计学意义(P<0.05);PDCA组多项监测指标的合格率明显提高(P<0.05)。结论手术室应用PDCA循环实现了预防在先的管理理念,为手术室院感的预防与控制提供了科学依据。

Objective To explore the role of PDCA circulation in controlling nosocomial infection in operating room.Methods The clinical data before and after PDCA circulation in operating room of Jingzhou hospital of traditional Chinese medicine from 2015 to 2018 was collected,and the patients were divided into routine group and PDCA group according to the management methods.Retrospective analysis was conducted on the effect of the two groups in the quality management of nosocomial infection in operating room.Results The scores of environmental management,disinfection and isolation management,safety management and drug management in PDCA group were all higher than those in routine group(P<0.01);the detection rate of pathogens in surgical incision was lower than that in routine group(P<0.01);the infection rate of surgical incision was lower than that in routine group(P<0.01);the infection rates of type I,type II and type III surgical incision in PDCA group were 1.14%,3.23%,7.02%,respectively,and all were decreased significantly.The difference between the two groups was statistically significant.(P<0.05);the qualified rate of multiple monitoring indicators in PDCA group was significantly increased(P<0.05).Conclusion The application of PDCA circulation in operating room realizes the management concept of prevention first,which provides scientific basis for the prevention and control of nosocomial infection in operating room.
