留置单针透析在动静脉内瘘狭窄球囊扩张术围术期的应用

Application of Indwelling Single Needle Dialysis in Perioperative Period of Balloon Dilatation for Arteriovenous Fistula Stenosis

目的:探讨留置单针穿刺法在血液透析患者经皮动静脉内瘘狭窄球囊扩张术围术期的使用效果。方法:选择我科经皮动静脉内瘘狭窄球囊扩张术患者64例分为观察组和对照组。对照组32例予以双针穿刺,观察组32例予以留置单针穿刺,比较两组球囊扩张术后皮下血肿,再次狭窄时间以及内瘘失功发生情况差异。结果:观察组皮下血肿发生率为6.25%,明显低于对照组的25.00%,一年内内瘘再狭窄发生率观察组为18.75%,低于对照组的28.13%,内瘘失功观察组为0,明显低于对照组的3.13%,差异有统计学意义(P<0.05)。结论:经皮动静脉内瘘狭窄球囊扩张术围术期使用留置单针穿刺法更有利于保护内瘘,延长使用寿命,减少穿刺次数,减轻患者痛苦。

Objective To explore the effect of single needle aspiration in the perioperative period of percutaneous arteriovenous fistula stenosis balloon dilatation in hemodialysis patients. Methods 64 patients with stenosis of arteriovenous fistulas in our department were divided into observation group and control group. 32 cases in the control group were treated with double needle puncture and 32 cases in the observation group were treated with single needle puncture. The differences of hematoma, time of restenosis and the occurrence of internal fistula failure between the two groups were compared. Results The incidence of subcutaneous hematoma in the observation group was 6.25%, which was significantly lower than that in the control group(25.00%) within one year. The incidence of internal fistula restenosis in the observation group was 18.75%, which was lower than that in the control group(28.13%).The incidence of internal fistula failure in the observation group was 0%, which was significantly lower than that of the control group(3.13%)(P<0.05). Conclusion In the perioperative period, the use of single needle puncture is more conducive to the protection of internal fistula, prolong the service life, reduce the number of puncture, and reduce the pain of patients.
