MMP-7在直肠癌中的表达及临床意义

Expression and Clinical Significance of MMP-7 in Rectal Cancer

目的探讨基质金属蛋白酶-7(MMP-7)在直肠癌中的表达及临床意义。方法选取2015年10月至2018年8月商丘市第一人民医院普外科确诊的96例直肠癌患者标本,在距癌组织远端切缘1、2、3 cm处分别横向剪取宽2 mm的黏膜组织作为手术切缘(分别标记为A组、B组、C组),根据分化程度的不同,高、中分化组分别标记为A1组、B1组、C1组,低分化组分别标记为A2组、B2组、C2组,选取50例轻微慢性直肠炎症患者黏膜组织标本作为对照组(D组)。采用免疫组化方法检测各组黏膜组织中MMP-7的表达水平。结果直肠癌组织中MMP-7阳性表达率高于直肠非癌组织,差异有统计学意义(P<0.05)。A1组、B1组MMP-7阳性表达率均高于C1组、D组,差异有统计学意义(均P<0.05);C1组和D组MMP-7阳性表达率比较,差异无统计学意义(P>0.05)。A2组MMP-7阳性表达率高于C2组,差异有统计学意义(P<0.05);A2组和B2组MMP-7阳性表达率均高于D组,差异有统计学意义(均P<0.05);B2组和C2组MMP-7阳性表达率比较,差异无统计学意义(P>0.05);C1组MMP-7阳性表达率低于C2组,差异有统计学意义(P<0.05);C2组MMP-7阳性表达率高于D组,差异有统计学意义(P<0.05)。结论直肠癌组织中MMP-7存在异常表达,随着与直肠癌距离的增加,MMP-7在组织中的表达呈下降趋势,且可能随着分化程度的不同而发生改变,可以作为判断直肠癌安全手术切缘的参考。

Objective To investigate the expression and clinical significance of matrix metalloproteinase-7(MMP-7)in rectal cancer.Methods A total of 96 cases of rectal cancer specimens confirmed by general surgery department in Shangqiu First People’s Hospital from October 2015 to August 2018 were selected.Two millimetres mucosal tissues transversely cut at 1,2 and 3 cm away from the distal margins of the cancer tissues were used as the surgical margins(group A,group B and group C were labeled,respectively).According to the degree of differentiation,the high and middle differentiated groups were labeled as A1 group,B1 group and C1 group,respectively,and the low differentiated groups were labeled as A2 group,B2 group and C2 group,respectively.Fifty cases of mucosa tissue specimens from patients with mild chronic rectal inflammation were selected as control group(group D).Immunohistochemistry was used to detect the expression levels of MMP-7 in mucosa tissues of each group.Results The positive expression rate of MMP-7 in rectal cancer tissues was higher than that in non-cancer tissues,and the difference was statistically significant(P<0.05).The positive expression rates of MMP-7 in A1 group and B1 group were higher than those in C1 group and D group,and the differences were statistically significant(all P<0.05).There was no significant difference in the positive expression rate of MMP-7 between C1 group and D group(P>0.05).The positive expression rate of MMP-7 in A2 group was higher than that in C2 group,and the difference was statistically significant(P<0.05).The positive expression rates of MMP-7 in A2 group and B2 group were higher than that in group D,and the differences were statistically significant(both P<0.05).There was no significant difference in the positive expression rate of MMP-7 between B2 group and C2 group(P>0.05).The positive expression rate of MMP-7 in C1 group was lower than that in C2 group,and the difference was statistically significant(P<0.05).The positive expression rate of MMP-7 in C2 group was higher than that in D group,and the difference was statistically significant(P<0.05).Conclusion There is an abnormal expression of MMP-7 in rectal cancer tissues.The expression level of MMP-7 decreased with the increased distance of rectal cancer,which may change as the degree of differentiation.MMP-7 may provide a reference for the safe surgical margin of rectal cancer.
