呼吸重症患者行幽门后营养支持治疗对医院获得性肺炎与病死率的影响研究

Study on the effect of post pylorus nutritional support on hospital acquired pneumonia and mortality in patients with severe respiratory disease

目的:分析幽门后营养支持治疗呼吸重症患者对医院获得性肺炎(HAP)及病死率的临床影响。方法:2018年2月-2019年2月收治呼吸重症患者72例,按营养支持方式不同分为常规经鼻胃管营养35例和幽门后营养支持37例。比较两组医院HAP发生率与病死率。结果:幽门后营养支持组HAP发生率显著低于常规鼻胃管组,差异有统计学意义(P<0.05);幽门后营养支持组呼吸机相关肺炎(VAP)发生率及RICU病死率虽略低于常规鼻胃管组,但差异无统计学意义(P>0.05)。结论:幽门后营养支持治疗呼吸重症患者可降低HAP发生率,但对VAP、RICU发生率无明显影响。

Objective:To explore the effect of post pylorus nutritional support on hospital acquired pneumonia and mortality in patients with severe respiratory disease.Methods:From February 2018 to February 2019,72 patients with severe respiratory disease were selected.According to the different ways of nutrition support,they were divided into 35 cases of routine nasogastric nutrition and 37 cases of post pylorus nutrition support.We compared the hospital HAP incidence and mortality between the two groups.Results:In the post pylorus nutritional support group,the incidence of HAP was significantly lower than that in the routine nasogastric tube group,and the difference was statistically significant(P<0.05).In the post pylorus nutritional support group,the incidence of VAP and mortality of RICU were slightly lower than those of the routine nasogastric tube group,and there was no significant difference(P>0.05).Conclusion:The incidence of HAP in patients with severe respiratory disease can be reduced by pylorus nutrition support,but there is no significant effect on the incidence of VAP and RICU.
