蚌埠市区甲状腺良性结节患者碘营养水平与甲状腺功能相关性分析 被引量:3

摘要 目的:观察蚌埠市区居民甲状腺良性结节患者碘营养状况,分析其与甲状腺功能的相关性,指导甲状腺良性结节患者的饮食结构。方法:选取在某院门诊就诊经彩超确诊的甲状腺良性结节患者260例为病例组,并选取同期在体检中心体检的无甲状腺结节的健康人群130例为对照组,采用过氧乙酸四甲基联苯胺氧化显色法测量尿碘,电化学发光法检测血清三碘甲状腺原氨酸(T3)、甲状腺素(T4)、血清游离三碘甲状腺原氨酸(FT3)、血清游离甲状腺素(FT4)及促甲状腺素(TSH)水平。结果:病例组与对照组尿碘水平比较差异无统计学意义(P>0.05),2组尿碘水平主要聚集在100~199μg/L,但2组碘营养状态构成比较差异有统计学意义(P<0.05);甲状腺良性结节组患者不同碘营养状态下TT3、TT4、FT3、FT4、TSH水平与对照组比较差异均无统计学意义(P>0.05);病例组及对照组尿碘与TT3、TT4、FT3、FT4、TSH水平无明显相关性(P>0.05)。结论:蚌埠市区居民整体处于碘适量状态,甲状腺良性结节患者不要盲目补碘或限制碘的摄入,应根据尿碘水平选择个体化的治疗方案。 Objective:To observe the iodine nutrition status of residents with benign thyroid nodules in Bengbu city and analyze its correlation with thyroid function so as to guide the dietary structure of patients with benign thyroid nodules.Methods:260 cases of thyroid benign nodules diagnosed by color Doppler ultrasound were selected as the case group. In addition, 130 healthy adults without thyroid nodules who underwent physical examination at the same time were selected as the control group. Urine iodine was measured by oxidation chromogenic method of tetramethylbenzidine peracetic acid. Serum triiodothyronine(T3), thyroxine(T4),serum free triiodothyronine(FT3), serum free thyroxine(FT4) and thyrotropin(TSH) were measured by electrochemical luminescence.Results:There was no significant difference in urine iodine level between the two groups(P>0.05).Urine iodine levels in the two groups were mainly concentrated between 100~199 μg/L.However, the difference in the composition of iodine nutritional status between the two groups was statistically significant(P<0.05). There was no significant difference in TT3, TT4, FT3, FT4,TSH levels between the the two groups under different iodine nutritional status(P>0.05).There was no significant correlation between urine iodine and TT3, TT4, FT3, FT4 TSH levels in the two groups(P>0.05).Conclusion:Residents in Bengbu city are in condition of appropriate iodine. Patients with benign thyroid nodules should not blindly supplement iodine or limit the intake of iodine, but should choose individualized treatment according to the urine iodine level.
