EndurantⅡ支架在复杂解剖结构腹主动脉瘤中的应用

Application of Endurant Ⅱ stent graft in patients with challenging abdominal aortic anatomies

目的探讨EndurantⅡ支架治疗复杂解剖结构腹主动脉瘤的有效性及安全性。方法回顾性分析重庆医科大学附属第一医院2017年2~10月采用EndurantⅡ支架治疗的17例复杂解剖结构腹主动脉瘤患者的临床资料,总结技术成功率、临床成功率及术后并发症发生率。结果17例患者均成功释放支架。Ⅰa型内漏2例,均于术后1个月随访时消失,总体技术成功率为88.2%,临床成功率为100%。围术期无死亡病例。随访15.6±2.3(12~20)个月,无迟发型内漏、瘤体增大、支架移位、断裂、狭窄、闭塞等发生。结论EndurantⅡ支架治疗复杂解剖结构腹主动脉瘤具有较高的有效性和安全性,但中远期结果仍需更大样本及长时间随访的研究观察。

Objective To evaluate the safety and effectiveness of the EndurantⅡstent-graft for abdominal aortic aneurysm patients with challenging anatomies.Methods The clinical data of 17 abdominal aortic aneurysm patients with challenging anatomies treated with EndurantⅡstent-graft in the First Affiliate Hospital of Chongqing Medical University between February 2017 and October 2017 was analyzed retrospectively.The technical success rate,clinical success rate and incidence of postoperative complications were summarized.Results Seventeen patients released the stent successfully.Two cases occured typeⅠa endoleak and disappeared at 1 month postoperatively.The overall technical success rate was 88.2%,and the clinical success rate was 100%.There were no deaths during the perioperative period.In the follow-up time of 15.6±2.3(12~20)months,no delayed endoleak,tumor enlargement,stent displacement,fracture,stenosis or occlusion occurred.Conclusion EndurantⅡstent-graft for abdominal aortic aneurysm patients with challenging anatomies is effective and safe.Further studies with larger sample and longer follow-up are needed to evaluate mid-and long-term results.
