调Q开关Nd:YAG激光对色素性皮肤病患者黑素代谢及皮损恢复的影响

Effect of Q-switched Nd: YAG laser on melanin metabolism and skin lesion recovery in patients with pigmented dermatosis

目的探讨调Q开关Nd:YAG激光治疗色素性皮肤病的有效性,分析该激光技术对患者黑素代谢与皮损恢复的影响。方法选择2016年1月至2018年1月医院皮肤科收治的色素性皮肤病患者200例为研究对象,根据随机数字表法分为对照组与研究组,对照组(100例)患者接受传统强脉冲光治疗,在此基础上为研究组(100例)患者联合实施调Q开关Nd:YAG激光治疗,治疗结束后随访6个月。治疗结束时,评价并对比两组患者临床疗效;分别于治疗前、治疗后3个月、治疗后6个月,检测并对比两组黑素代谢指标[褪黑素(MLT)、酪氨酸酶(TYE)、α-黑素细胞刺激素(α-MSH)]及皮损恢复情况。结果治疗结束时,研究组临床有效率较对照组高,差异有统计学意义(P<0.05);治疗后各时点,研究组血清TYE、α-MSH表达较对照组降低,血清MLT水平较对照组升高,差异有统计学意义(P<0.05);治疗后各时点,研究组斑块颜色、斑块面积评分均较对照组降低,差异有统计学意义(P<0.05)。结论调Q开关Nd:YAG激光治疗色素性皮肤病疗效确切,更利于改善患者黑素代谢,帮助其皮损更好、更快恢复,值得推广应用。

Objective To investigate the effectiveness of Q-switched Nd:YAG laser in treatment of pigmented dermatosis,and to analyze its effect on melanin metabolism and skin lesion recovery.Methods 200 patients with pigmented dermatosis in the department of dermatology from January 2016 to January 2018 were selected as the subjects,and they were divided into control group and research group by random number table.Control group(100 cases)received traditional intense pulsed light therapy,and research group(100 cases)was treated with Q-switched Nd:YAG laser based on control group.They were given 6 months of follow-up after treatment.At the end of treatment,the clinical efficacy of two groups was evaluated and compared;Before treatment,after treatment for 3 months and 6 months,the melanin metabolism indexes[melatonin(MLT),tyrosinase(TYE),α-melanocyte stimulating hormone(α-MSH)],skin lesion recovery in two groups were detected and compared.Results At the end of treatment,the clinical effective rate of research group was higher than that of control group(P<0.05);At each time after treatment,the levels of serum TYE andα-MSH in research group were lower than those in control group,and the level of MLT in research group was higher than that in control group(P<0.05);At each time after treatment,the color of patch and the score of patch area in research group were lower than those in control group(P<0.05).Conclusion Q-switched Nd:YAG laser in treatment of pigmented dermatosis is effective,it can improve melanin metabolism and promote skin lesion recovery,which is worthy of promotion and application.
