TCT在甲状腺细针穿刺病理诊断中的应用

摘要 目的分析液基膜式薄层细胞学技术(thin prepliquid based cytology testing,TCT)应用于B超引导下甲状腺细针穿刺病理诊断的临床效果。方法选取3 376例2018年1月—2019年1月在本院进行甲状腺细针穿刺病理检查的患者,对所采集的标本均进行常规细胞涂片制片(cytological smear,CS)和TCT制片,比较两种方法的诊断效果。结果 TCT标本中不满意标本占比17.0%,显著低于CS标本的19.3%,差异有统计学意义(P <0.05)。TCT与CS标本的Ⅱ级、Ⅲ级、Ⅳ级和Ⅴ级标本所占比重比较,差异均无统计学意义(P> 0.05)。TCT标本的阳性检出率为26.9%,CS标本的阳性检出率为25.5%,组间差异无统计学意义(P> 0.05)。结论 B超引导下甲状腺细针穿刺采用TCT制片,具有较高的涂片质量,且诊断准确率高,在甲状腺疾病诊断中具有重要应用价值。 Objective To analyze the clinical effect of B-mode ultrasound-guided liquid-based membrane thin layer cytology(TCT)in the pathological diagnosis of thyroid fine needle puncture.Methods 3376 patients who underwent fine needle aspiration pathology in our hospital from January 2018 to January 2019 were selected.The samples were taken by routine cell smear and TCT,and the diagnostic effects of the two methods were compared.Results The proportion of unsatisfactory specimens in TCT specimens was 17.0%,which was significantly lower than that in CS specimens(19.3%),the difference was statistically significant(P<0.05).There was no significant difference in the proportion ofⅡ,Ⅲ,ⅣandⅤspecimens between TCT and CS specimens(P>0.05).The positive rate of TCT specimens was 26.9%,and that of CS specimens was 25.5%.There was no significant difference between the two groups(P>0.05).Conclusion Thyroid fine needle puncture guided by B-mode ultrasound is made by TCT,which has high smear quality and high diagnostic accuracy.It has important application value in the diagnosis of thyroid diseases.
