sRAGE及esRAGE与原发性高血压合并糖代谢异常的相关性 被引量:2

摘要 目的研究可溶性糖基化终产物受体(soluble receptor of advanced glycation end products,sRAGE)及内源性分泌型糖基化终产物受体(endogenous secretory receptor of advanced glycation endproducts,esRAGE)与原发性高血压(essential hypertension,EH)伴发糖代谢异常(impaired glucose metabolism,IGM)的相关性。方法筛选856例正常糖耐量的EH患者(年龄55~70岁),随访3年。采用酶联免疫吸附试验(ELISA)方法测定患者循环血esRAGE及sRAGE浓度。结果3年后有167例进展为空腹血糖受损(19.5%),248例进展为糖耐量受损(29.0%),15例进展为糖尿病(1.8%)。EH+IGM组血浆esRAGE和sRAGE浓度明显低于EH control组,差异有统计学意义(P<0.05)。进一步的Logistic回归分析表明,血浆esRAGE和sRAGE下降每100 pg/ml分别会使IGM的风险增加2.26倍(95%可信区间:1.73~2.82,P<0.001)和2.11倍(95%可信区间:1.64~2.56,P<0.001)。结论esRAGE及sRAGE浓度减低是EH患者伴发IGM的独立危险因素,esRAGE和sRAGE浓度对EH患者发生IGM具有良好的诊断价值。
