透穴针刺法配合TDP照射在脑梗死后中枢性疼痛患者中的应用研究

Application of point-through-point acupuncture combined with TDP irradiation in patients with central pain after cerebral infarction

目的探讨透穴针刺法配合神灯(TDP)照射在脑梗死后中枢性疼痛患者中的作用效果。方法选取2017年6月—2018年7月本院收治的88例脑梗死后中枢性疼痛患者作为研究对象,采用随机数表法将其分为对照组和研究组两组,每组各44例。对照组实施TDP照射疗法,研究组实施透穴针刺配合TDP照射疗法。采用McGill疼痛问卷(简表)(SF-MPQ)评估治疗前后两组患者的疼痛状况;对比两组患者的临床疗效;采用Zung抑郁自评量表(SDS)、匹斯堡睡眠质量指数量表(PSQI)评估并对比治疗前后两组患者的抑郁状态和睡眠质量;评价两组治疗方法的安全性。结果治疗前两组PRI评分中感觉、情感及总分、VAS、PPI评分及SF-MPQ总分比较,差异均无统计学意义(P>0.05),治疗后研究组与对照组相比,上述评分均较低(P<0.05);治疗后与治疗前相比,两组上述评分均降低(P<0.05);两组临床疗效等级分布比较,差异有统计学意义(P<0.05),且研究组治疗总有效率高于对照组(P<0.05)。治疗前两组SDS、PSQI评分比较,差异均无统计学意义(P>0.05),治疗后研究组与对照组相比,上述评分均较低(P<0.05);治疗后与治疗前相比,两组上述评分均降低(P<0.05);治疗期间研究组不良反应发生率6.82%,与对照组的4.55%相比,差异无统计学意义(P>0.05)。结论透穴针刺法配合TDP照射可显著缓解脑梗死后中枢性疼痛患者疼痛状况,疗效显著,且有助于改善患者抑郁状态和睡眠质量,安全可靠。

Objective To investigate the effect of point-through-point acupuncture combined with magic lamp(TDP)irradiation on patients with central pain after cerebral infarction.Methods 88 patients with central pain after cerebral infarction admitted to our hospital from June 2017 to July 2018 were randomly divided into control group and research group with 44 cases in each group.The control group was treated with TDP irradiation,and the research group was treated with point-through-point acupuncture combined with TDP irradiation.McGill pain questionnaire(SF-MPQ)was used to evaluate and compare the pain status of the 2 groups before and after treatment.The clinical effects of two groups were compared.The depressive state and sleep quality of the two groups before and after treatment were compared and evaluated by Zung Self-rating Depression Scale(SDS)and Pittsburgh Sleep Quality Index(PSQI).Safety of two groups of treatment was evaluated.Results There were no significant differences in scores of sensation,emotion and PRI total score,VAS,PPI score and SF-MPQ score between the two groups before treatment(P>0.05),while the above scores of the research group after treatment were lower than those of the control group(P<0.05).Compared with those before treatment,the above scores in both groups were decreased after treatment(P<0.05).There was significant difference in the clinical efficacy grade distribution between the two groups(P<0.05),and the total effective rate of the research group was higher than that of the control group(P<0.05).There were no significant differences in SDS and PSQI scores between the two groups before treatment(P>0.05),compared with the control group,the above scores in the research group were lower after treatment(P<0.05),compared with those before treatment,the above scores in both groups were decreased after treatment(P<0.05).During the treatment,the incidence of adverse reactions in the research group was 6.82%,and there was no significant difference between the research group and the control group(4.55%),P>0.05.Conclusions Acupuncture combined with TDP irradiation could significantly alleviate the pain of patients with central pain after cerebral infarction,and the curative effect is significant,and it also could help to improve the depression and sleep quality of patients,it is safe and reliable.
