摘要
目的探讨体表心电图计算LVEF的方法,在不同左心室构型高血压病患者中应用价值。方法方便选取2018年9—11月在福建医科大学附属福州市第一医院心血管内科门诊治疗的110例高血压病患者。根据左心室不同构型分为正常构型组、离心性肥厚组和向心性重构组。观察各组通过体表心电图计算出的LVEF值及其超声心动图LVEF值,并对两者数值进行比较。结果除了离心性肥厚组[(50.3±6.1)%vs(54.7±6.4)%,P=0.021],在正常构型组[(53.9±8.2)%vs(54.0±7.8)%,P=0.291]、向心性重构组[(52.9±8.3)%vs(53.0±7.8)%,P=0.898],患者中体表心电图计算出的LVEF值与超声心动图中的LVEF值差异无统计学意义(P>0.05)。在离心性肥厚组,体表心电图计算出的LVEF数值与超声心动图中的LVEF数值不存在相关性(r=0.355,P=0.163),而在正常构型组、向心性重构组中两种方法测得的LVEF数值正相关(P<0.05)。结论除了离心性肥厚组,正常构型组、向心性重构组高血压病患者可以通过体表心电图计算LVEF值。
Objective To explore the method of calculating LVEF by surface electrocardiogram,and to apply it in patients with different left ventricular configuration hypertension.Methods 110 patients with hypertension who were treated in the Department of Cardiology,Fuzhou First Hospital,Fujian Medical University from September to November 2018.According to different configurations of left ventricle,it was divided into normal configuration group,centrifugal hypertrophy group and centripetal reconstruction group.The LVEF values calculated by the surface electrocardiogram and the echocardiogram LVEF values of each group were observed,and the values were compared.Results Except for the eccentric hypertrophy group[(50.3±6.1)%vs(54.7±6.4)%,P=0.021],in the normal configuration group[(53.9±8.2)%vs(54.0±7.8)%,P=0.291],centripetal remodeling group[(52.9±8.3)%vs(53.0±7.8)%,P=0.898],the difference between the LVEF value calculated by the surface electrocardiogram and the LVEF value in the echocardiogram was not statistically significant(P>0.05).In the eccentric hypertrophy group,there was no correlation between the LVEF values calculated by the surface electrocardiogram and the LVEF values in the echocardiogram(r=0.355,P=0.163).In the normal configuration group and the centripetal remodeling group,the LVEF values measured by the two methods were positively correlated(P<0.05).Conclusion In addition to the eccentric hypertrophy group,patients with hypertension in the normal configuration group and the centripetal remodeling group can calculate the LVEF value by surface electrocardiogram.
