呼吸功能训练联合早期肠内营养对重症脑血管病患者呼吸功能和意识状态的影响 被引量:1

Study on the Effect of Respiratory Function Training Combined with Early Enteral Nutrition on Respiratory Function and Consciousness in Patients with Severe Cerebrovascular Disease

摘要 目的探究呼吸功能训练联合早期肠内营养对重症脑血管病患者呼吸功能和意识状态的影响。方法方便收集该院于2017年1月—2018年12月收治的重症脑血管病患者56例,随机分为对照组和试验组,各28例。对照组患者予以常规康复训练联合早期营养支持,康复锻炼2次/d,30 min/次;试验组在对照组的基础上予以呼吸功能训练联合早期营养支持,30 min/次,2次/d,每周进行5次呼吸锻炼。两组均连续治疗4周。对比分析两组呼吸功能以及意识状态。结果治疗后两组TV、呼吸频率以及心率低于治疗前,治疗后肺活量、最长呼气时间、GCS分高于治疗前,差异有统计学意义(P<0.05);治疗后试验组TV(403.10±42.90)mL低于对照组,试验组肺活量(166.39±20.22)V/mL、最长呼气时间(7.29±0.79)t/s、GCS分(12.28±1.52)分高于对照组,差异有统计学意义(t=20.186、18.324、9.603、5.124、P<0.05)。结论呼吸功能训练联合早期肠内营养干预后重症脑血管病患者的呼吸功能及意识状态显著改善,潮气量降低,肺活量、最长呼气时间提高,需要进一步扩大样本以进行进一步探究。 Objective To explore the effects of respiratory function training combined with early enteral nutrition on respiratory function and consciousness in patients with severe cerebrovascular disease.Methods 56 patients with severe cerebrovascular disease from January 2017 to December 2018 in the hospital were randomLy divided into control group and experimental group,28 cases each.Patients in the control group were given routine rehabilitation training combined with early nutritional support,rehabilitation exercise twice a day,30 minutes each time;patients in the experimental group were given respiratory function training combined with early nutritional support on the basis of the control group,30 minutes each time,twice a day,five breathing exercises per week.Both groups were treated continuously for 4 weeks.Respiratory function and consciousness were compared between the two groups.Results After treatment,the TV,respiratory rate and heart rate of the two groups were lower than those before treatment.The lung capacity,the longest expiratory time and the GCS score were higher than those before treatment the difference was statistically significant(P<0.05).After treatment,the TV group(403.10±42.90)mL was lower than the treatment group.In the control group,the lung capacity(166.39±20.22)V/mL,the longest expiratory time(7.29±0.79)t/s,and the GCS score(12.28±1.52)points were higher in the experimental group than in the control group,and the difference was statistically significant(t=20.186,18.324,9.603,5.124,P<0.05).Conclusion Respiratory function training combined with early enteral nutrition intervention can significantly improve the respiratory function and consciousness of patients with severe cerebrovascular disease,reduce tidal volume,increase vital capacity and maximal expiratory time,and further enlargement of samples is needed for further exploration.
