优质抗凝门诊对心脏瓣膜置换术后患者抗凝治疗依从性的影响

Effect of High-quality Anticoagulant Clinic on the Compliance of Patients with Anticoagulant Therapy after Cardiac Valve Replacement

目的探讨优质抗凝门诊对心脏瓣膜置换术后患者抗凝治疗依从性的影响。方法采用目的抽样方法,抽取2015年1月—2016年12月在该院心血管外科门诊就诊心脏瓣膜置换术后患者320例,按照随机数字表法将患者分为观察组和对照组,各160例,对照组由主管医生进行常规诊疗,观察组由优质抗凝门诊人员进行干预,包括建立个人信息档案、进行抗凝治疗管理和生活方式的咨询指导。采用自行设计抗凝治疗相关知识调查表、抗凝治疗依从性调查表,比较两组患者的抗凝依从性。结果观察组与对照组患者一般资料比较差异无统计学意义(P>0.05),观察组完全了解抗凝治疗相关知识的患者112例,占比71.34%,对照组为51.61%,两组相比差异有统计学意义(χ^2=12.821,P=0.003)。观察组1年期抗凝治疗依从性较好的患者共116例,占比73.88%,对照组为52.26%,两组差异有统计学意义(χ^2=14.710,P=0.000)。结论优质抗凝专科门诊能提高心脏瓣膜置换术后患者的抗凝治疗依从性,减少并发症的发生。

Objective To investigate the effect of high-quality anticoagulant clinic on the compliance of patients with anticoagulant therapy after cardiac valve replacement.Methods From January 2015 to December 2016,320 patients with cardiac valve replacement in the cardiovascular surgery clinic were divided into observation group and control group according to random number table.The control group was given routine diagnosis and treatment by the competent doctors,and the observation group was given intervention by the high-quality anticoagulant clinic staff,including the establishment of personal information files and the implementation.Consultation and guidance on management and lifestyle of anticoagulation therapy.The self-designed questionnaire of knowledge about anticoagulation therapy and the compliance questionnaire of anticoagulation therapy were used to compare the anticoagulation compliance between the two groups.Results There was no significant difference in general data between the observation group and the control group the difference was not statistically significant(P>0.05).112 patients in the observation group fully understood the knowledge of anticoagulation therapy,accounting for 71.34%,and 51.61%in the control group.There was a statistically significant difference between the two groups(χ^2=12.821,P=0.003).116 patients in the observation group had good compliance with anticoagulation therapy in one year,accounting for 73.88%,compared with 52.26%in the control group.There was a statistically significant difference between the two groupsthe difference(χ^2=14.710,P=0.000).Conclusion High-quality anticoagulant clinic can improve the compliance of patients with anticoagulant therapy after heart valve replacement and reduce the incidence of complications.
