前弹力层下激光角膜磨镶术后眼前节结构的变化

Changes of anterior segment structure after sub-bowman keratomileusis

目的:观察前弹力层下激光角膜磨镶术(SBK)对角膜后表面曲率、后表面高度和前房参数等眼前节结构的影响。方法:前瞻性自身对照研究。接受SBK手术矫正近视患者49例97眼,采用Oculyzer眼前节分析系统测量术前和术后1wk,1、3a的角膜后表面平均曲率、中央角膜后表面高度(PCE)、前房容积(ACV)、中央和周边前房深度(ACD)。应用重复测量方差分析比较手术前后各参数的差异。结果:SBK术后各时间点角膜后表面平均曲率与术前比较无差异(P>0.05)。而术后1wk,1、3a的中央PCE、ACV和中央ACD较术前均有所减小(P<0.01)。除术后3a上方周边ACD外,术后各时间点其余各方位周边ACD较术前均有所减小(P<0.05)。结论:SBK术后角膜后表面曲率保持稳定,中央PCE轻度后移,而前房整体变浅。

·AIM:To observe the effect of sub-bowman keratomileusis(SBK) on the posterior corneal curvature,posterior corneal elevation and anterior chamber parameters·METHODS:Prospective self controlled study.Totally 49 patients(97 eyes) with myopia who had planned for SBK were included in this study,mean posterior corneal curvature,central posterior corneal elevation(PCE),anterior chamber volume(ACV),central and peripheral anterior chamber depth(ACD) were measured by Oculyzer before and 1 wk,1y,3y after surgery.Repeated measure analysis of variance was adopted for data analysis.·RESULTS:There were no significant differences in mean posterior corneal curvature at any time point postoperatively compared with that preoperatively(P>0.05).However,central PCE,ACV and central ACD at 1 wk,1y and 3y postoperatively were all decreased(P<0.01).Except for the anterior peripheral ACD at 3 y after SBK,other peripheral ACD at any time point postoperatively were all decreased(P<0.05).·CONCLUSION:The posterior corneal curvature remained stable after SBK,while the central PCE shifted slightly backward and the entire anterior chamber became shallow.
