摘要
目的:探究胃蛋白酶原Ⅰ、Ⅱ(PGⅠ、PGⅡ)及胃泌素17(G-17)联合14-碳素(14C)呼气幽门螺旋杆菌(Hp)检测在早期胃癌筛查中的应用。方法:选取2018年4月至2019年6月在我院进行胃癌筛查的胃癌高危人群569例。根据血清PGⅠ、PGⅡ、G-17检测以及Hp试验检查结果将所有患者分为A、B、C、D四组。根据胃镜检查结果将受试者分为浅表性胃炎、萎缩性胃炎、胃癌组。以胃镜检查为标准,比较A、B、C、D四组胃癌检出率;比较浅表性胃炎、萎缩性胃炎、胃癌三组的血清PGⅠ、PGⅡ、G-17含量。结果:以胃镜检查为标准,C组患者胃癌检出率高于A、B、D组(P<0.05);胃癌组血清PGⅠ、PGⅡ、G-17含量高于浅表性胃炎、萎缩性胃炎组。结论:G-17、PGⅠ、PGⅡ和Hp联合检测应用于早期胃癌筛查中具有重要意义,可为胃癌早期诊断提供有效依据。
Objective: To investigate the application of pepsinogen(PG) 1, 2 and gastrin 17(G-17) combined with 14-carbon(14C) expiratory Helicobacter pylori(Hp) in early gastric cancer screening. Methods: A total of 569 high-risk patients with gastric cancer who underwent gastric cancer screening in the hospital from April 2017 to April 2019 were enrolled. They were into A, B, C, and D group according to serum PGⅠ, PGⅡ, G-17 and Hp results. They were into superficial gastritis group, atrophic gastritis group and gastric cancer group according to gastroscopy results. Taking gastroscopy as standard, the detection rate of gastric cancer was compared among group A, B, C and D. The contents of serum PGⅠ, PGⅡ and G-17 were compared among superficial gastritis group, atrophic gastritis group and gastric cancer group. Results: Taking gastroscopy as standard, the detection rate of gastric cancer in group C was higher than that in group A, B and D(P<0.05). The contents of serum PGⅠ, PGⅡ and G-17 in gastric cancer group were higher than those in superficial gastritis group and atrophic gastritis group. Conclusion: The application of combination detection with G-17, PGⅠ, PGⅡ and HP is of important significance in early gastric cancer screening, which can provide effective basis for early diagnosis of gastric cancer.
