SREBP1在不同分子分型乳腺癌中的表达研究及临床意义

摘要 背景与目的:中国是乳腺癌发病率增长速度全世界最快的国家之一,癌症负担在中国不断增加。调查显示乳腺癌的发生发展与体内脂质代谢密切相关,固醇调节元件结合蛋白(sterol regulatory element binding transcription proteins,SREBPs)是真核细胞内调节脂类代谢的关键核转录因子,有研究表明SREBP1和其靶基因在大多数癌症中过度表达,且其表达程度与许多恶性肿瘤的恶性转化、进展和转移具有一致性,本研究的目的是探讨SREBP1在不同分子分型乳腺癌中的表达研究及临床意义。方法:收集2017年10月~2019年01月已由佳木斯大学附属第一医院病理科明确诊断为乳腺癌的手术切除标本120例,在此120例标本中随机抽取其中癌旁3 cm正常组织(镜下无癌)80例,通过免疫组化染色方法检测SREBP1在乳腺癌及癌旁组织中的表达情况。结果 SREBP1在乳腺癌组织中的表达与乳腺癌患者的年龄和肿块大小及组织学分级上无明显统计学意义(P>0.05),而在乳腺癌有无淋巴结转移和临床TNM分期上具有统计学意义(P<0.05);SREBP1在乳腺癌中的表达与PR、Ki67和Her2存在着正相关关系(P<0.05,rs>0),而与ER则无相关性。SREBP1在不同分子分型乳腺癌中的表达差异具有显著的统计学意义(χ~2=15.20,P<0.01)。结论:关于SREBP1在不同分子分型乳腺癌中的表达研究较少,有望通过SREBP1的检测,为乳腺癌的分子分型提供又一参考指标,来指导个体化治疗及判断预后。目前,抗脂代谢药物已经在临床上用于治疗肥胖。通过本研究有助从癌细胞的生物学和代谢等角度开拓出乳腺癌诊断与治疗的新思路。 Background and Objective: China is one of the fastest growing countries in the world for breast cancer incidence, and the burden of cancer is increasing in China. The investigation shows that the occurrence and development of breast cancer is closely related to lipid metabolism in the body. sterol regulatory element binding transcription proteins(SREBPs) are key nuclear transcription factors regulating lipid metabolism in eukaryotic cells. Studies have shown that SREBP1 And its target gene is over expressed in most cancers, and its expression level is consistent with the malignant transformation, progression and metastasis of many malignant tumors. The purpose of this study is to investigate the expression of SREBP1 in different molecular breast cancers. Clinical significance. Methods: A total of 120 surgically resected specimens from the Department of Pathology of the First Affiliated Hospital of Jiamusi University from October 2017 to January 2019 were collected. Among the 120 specimens, 3 cm normal tissues were randomly selected. Eighty cases without cancer were detected by immunohistochemical staining for the expression of SREBP1 in breast cancer and adjacent tissues. Results: The expression of SREBP1 in breast cancer tissues and age and mass size and histological grade of breast cancer patients There was no statistically significant difference(P>0.05), but there was statistical significance in the presence or absence of lymph node metastasis and clinical TNM staging in breast cancer(P<0.05). The expression of SREBP1 in breast cancer was associated with PR, Ki67 and Her2. Positive correlation(P<0.05, rs>0), but no correlation with ER. The expression difference of SREBP1 in different molecular breast cancers was statistically significant(χ~2=15.20, P<0.01). Conclusion: There is little research on the expression of SREBP1 in different molecular breast cancers. It is expected to be detected by SREBP1 for breast cancer. Molecular typing provides another reference indicator to guide individualized treatment and to judge prognosis. Currently, anti-lipid metabolism drugs have been used clinically to treat obesity. Through this research, it will help to develop new ideas for the diagnosis and treatment of breast cancer from the perspective of the biology and metabolism of cancer cells.
