乳腺导管扩张症介入性药物灌注治疗及疗效随访

Interventional Drug Perfusion Therapy And Therapeutic Effect Follow-up Of Mammary Ductal Ectasia

目的:分析乳腺导管扩张症药物灌注治疗的疗效,探讨此治疗方法的可行性。材料与方法:对46例经溢液病理涂片及内分泌测定排除肿瘤性及乳腺外型溢液,再经导管造影技术诊断为乳腺导管扩张症的患者,排除禁忌症后,运用优化乳腺导管造影插管技术对溢液乳孔行介入性药物灌注治疗,隔日一次,4次为一疗程。每例灌注治疗后分别在6个月,12个月,24个月跟踪随访。结果:46例药物灌注治疗患者,治疗结束后41例溢液停止,总有效率达91.23%,效果显著。对治疗后溢液完全停止的41例进行6个月随访,3例复发,有效率92.68%,12个月随访,4例复发,有效率90.24%,24个月随访4例复发,有效率90.24%,药物灌注治疗后溢液未停止的5例患者中3例进行手术治疗,2例放弃治疗。结论:运用优化乳腺导管造影插管技术对诊断明确的乳腺导管扩张症进行介入性药物灌注治疗疗效显著,复发率较低,具有临床推广应用价值。

Purpose:To analyze the interventional treatment of drug infusion therapeutic effect of mammary duct ectasia and explore the feasibility of the treatment.Materials and methods:46 patients with mammary ductal ectasia diagnosed by catheterization which excluded oncological and extramammary nipple discharge through spill pathological smear and endocrine determination.The spill milk holes were under interventional drug perfusion therapy with the technology of optimized breast catheterization except the one who had contraindications,every other day,4 times for a course of treatment.All cases were followed up at 6,12,24 months respectively after perfusion treatment.Results:There were 41 cases among 46 patients with drug perfusion treatment who stopped nipple discharge.The total effective rate was 91.23%with remarkable effect.The 41 cases who stopped nipple discharge were followed up for 6,12,24 months respectively,There were 3,4,4 cases recurrence with effective rate 92.68%,90.24%and 90.24%respectively.Among 5 cases who still had nipple discharge,3 cases of them received surgical treatment but 2 gave up to continue treatment.Conclusion:The effect of interventional drug perfusion in the diagnosable mammary ductal ectasia is significant by using the optimized breast catheterization and the recurrence is low.It has clinical application value.
