连续肾脏替代治疗在急性失代偿性心力衰竭的临床研究

Clinical study of continuous renal replacement therapy in acute decompensated heart failure

目的对连续性肾脏替代疗法在急性失代偿性心力衰竭治疗中应用效果进行探究。方法随机选取2018年1月至2019年1月本院接诊的76例急性失代偿性心力衰竭患者为研究对象,根据电脑随机法分为对照组与研究组,各38例。对照组予以常规治疗方法,研究组在此基础上加入连续肾脏替代疗法,比较两组治疗效果。结果治疗后,研究组的清除液体量、平均液体清除效率高于对照组,对比差异有统计学意义(P<0.05);研究组治疗24 h NT-proBNP、48 h肌酐及NT-proBNP水平低于对照组,对比差异有统计学意义(P<0.05);研究组ICU治疗时间短于对照组(P<0.05),但两组住院时间、30 d再住院率、急诊处理率比较差异无统计学意义。结论对急性失代偿性心力衰竭患者予以连续肾脏替代疗法,有助于控制病情,改善心功能及肾功能,减短ICU治疗时间,值得优选。

Objective To investigate the effect of continuous renal replacement therapy in the treatment of acute decompensated heart failure.Methods A total of 76 patients with acute decompensated heart failure who were admitted to our hospital from January 2018 to January 2019 were randomly selected.They were divided into the control group and the study group according to the computer randomization method,with 38 cases in each group.The conventional treatment method was adopted,and the research group added continuous renal replacement therapy on this basis to compare the therapeutic effects of the two groups.Results After treatment,the total clearance fluid and average fluid clearance efficiency were higher than the control group,and the difference was significant(P<0.05).The level of NT-proBNP,48 h creatinine and NT-proBNP was lower in the study group than in the control group,the difference was significant(P<0.05).The ICU treatment time in the study group was shorter than that in the control group(P<0.05),but there was no significant difference in hospitalization time,30-day rehospitalization rate and emergency treatment rate between the two groups.Conclusion Continuous renal replacement therapy for patients with acute decompensated heart failure can help control the disease,improve heart function and renal function,and shorten the treatment time of ICU.It is worthy of preference.
