腹腔镜下微创手术对老年胃穿孔的疗效及对胃肠功能和生活质量的影响

Effect of laparoscopic minimally invasive surgery on gastrointestinal function and quality of life in elderly patients with gastric perforation

目的探讨腹腔镜下微创手术对老年胃穿孔的疗效以及对肠胃功能、生活质量的影响。方法选择2017年1月至2019年1月本院行腹腔镜微创手术的45例老年胃穿孔患者为观察组,同期行开放性手术的45例患者为对照组。统计两组患者肠鸣音恢复时间、首次排气时间、术后2 h、24 h、72 h时胃泌素(GAS)含量、手术并发症及生活质量评分。结果观察组的肠鸣音恢复比对照组快,术后排气时间比对照组短,并发症比对照组少,差异有统计学意义(P<0.05);两组患者术前及术后2 h血清中胃泌素(GAS)水平比较差异无统计学意义,观察组术后24 h胃泌素水平高于对照组(P<0.05);观察组术后生活质量高于对照组(P<0.05)。结论腹腔镜下微创手术对老年胃穿孔的疗效比传统开放性手术好,有助于患者更快更好的恢复肠胃功能和生活质量,还能降低手术并发症的出现,应进行推广学习。

Objective To investigate the effect of laparoscopic minimally invasive surgery on gastric perforation in the elderly and its influence on gastrointestinal function and quality of life.Methods 45 patients who underwent laparoscopic minimally invasive surgery in our hospital from January 2017 to January 2019 were selected as the observation group and 45 patients who underwent open surgery in the same period as the control group.The recovery time of bowel sounds,the first exhaust time,the gastrin(GAS)content at 2 h,24 h and 72 h after operation,the complications of operation and the quality of life score were counted.Results The recovery of bowel sounds in the observation group was faster than that in the control group,the exhaust time was shorter and the complications were fewer than that in the control group(P<0.05).There was no significant difference in serum gastrin(GAS)levels before and 2 hours after operation.There was significant difference in gastrin levels 24 hours after operation.The quality of life in the observation group was higher than that in the control group(P<0.05),and the quality of life in the observation group was higher than that in the control group(P<0.05).Conclusion The effect of laparoscopic minimally invasive surgery on gastric perforation in the elderly is better than that of traditional open surgery,which can help patients recover gastrointestinal function and quality of life faster and better,and reduce the occurrence of surgical complications.It should be popularized and studied.
