葡萄糖酸锌联合维生素AD辅助治疗对不同病原菌感染肺炎儿童免疫功能及临床疗效的影响

The effect of zinc gluconate combined with vitamin AD adjuvant therapy on immune function and clinical efficacy in children with pneumonia caused by different pathogens

目的探讨葡萄糖酸锌联合维生素AD辅助治疗对肺炎儿童免疫功能及临床疗效的影响。方法选取我院收治的不同病原菌感染肺炎患儿80例,根据治疗方案的不同,将采用葡萄糖酸锌联合维生素AD辅助治疗的患儿作为观察组,仅采用常规抗感染、对症治疗等治疗的患儿作为对照组,比较两组治疗前后体液免疫指标IgA、IgM、IgG、细胞免疫指标CD4^+、CD8^+、CD4^+/CD8^+的变化。结果细菌性肺炎(BP)、病毒性肺炎(VP)和肺炎支原体感染(MP)中观察亚组患儿治疗后CD4+、CD4+/CD8+细胞免疫指标和IgA、IgG、IgM体液免疫指标均较对照亚组明显升高,CD8+较对照亚组明显降低(P<0.05),其中治疗后MP观察亚组患儿上述指标变化幅度相比BP、VP明显增加(P<0.05)。观察组3个月内呼吸系统感染再发率明显低于对照组(χ2=4.713,P=0.030)。结论在对症治疗基础上加以葡萄糖酸锌和维生素AD进行辅助治疗对于不同病原菌感染肺炎儿童免疫功能的提高均有显著的作用,其中MP患儿作用尤为突出。

Objective To investigate the effect of zinc gluconate combined with vitamin AD adjuvant therapy on immune function and clinical efficacy of children with pneumonia.Methods We selected 80 children with pneumonia caused by different pathogenic pathogens as the research objects.According to the different treatment regiments,children treated with zinc gluconate combined with vitamin AD were designed as observation group,and children treated with routine anti-infection and symptomatic treatment were designed as control group.The changes of humoral immunity indexes such as IgA,IgM and IgG and cellular immunity indexes such as CD4+,CD8+and CD4+/CD8+before and after treatment were compared between the two groups.Results The levels of CD4+,CD4+/CD8+and IgA,IgG and IgM of bacterial pneumonia(BP),viral pneumonia(VP)and mycoplasma pneumonia infection(MP)in the observation group were significantly higher than those in the control group after treatment,and the level of CD8+was significantly lower than that in the control group(P<0.05).Among different pneumonias,the changes of the above indexes in MP observation group were significantly increased than those in BP and VP after treatment(P<0.05).The recurrent rate of respiratory infection in the observation group was lower than that in the control group within 3 months(χ2=4.713,P=0.030).Conclusion The zinc gluconate and vitamin AD as adjuvant therapy on the basis of symptomatic treatment has significant effects on the improvement of immune function in children with pneumonia caused by different pathogens,especially in MP children.
