摘要
目的研究神经肌肉电刺激疗法(NMES)联合吞咽功能训练治疗脑卒中(CAV)后吞咽障碍的效果。方法选取2018年2月~2019年2月我院收治的68例CAV后吞咽障碍患者,按照随机数字表法分为试验组和对照组,每组各34例。试验组采用NEMS疗法联合吞咽功能训练,对照组仅采用吞咽功能训练,比较两组的治疗总有效率、治疗前后舌骨喉复合体动度情况。结果通过洼田饮水试验,试验组治疗总有效率高于对照组,差异有统计学意义(P<0.05);两组治疗前舌骨喉复合体动度比较,差异无统计学意义(P>0.05);试验组治疗后舌骨上移和前移均大于治疗前,且大于对照组,差异有统计学意义(P<0.05);对照组治疗后舌骨上移和前移与治疗前比较,差异无统计学意义(P>0.05);两组治疗前后甲状软骨上移和前移比较差异无统计学意义(P>0.05)。结论NMES疗法联合吞咽功能训练可明显改善CAV后吞咽障碍患者的吞咽功能,可在临床推广应用。
Objective To study the effect of neuromuscular electrical stimulation therapy(NMES)combined with swallowing function training on dysphagia after stroke(CAV).Methods A total of 68 patients with post-CAV dysphagia from February 2018 to February 2019 were selected and divided into the experimental group and the control group according to random number table method,34 cases in each group.The experimental group treated with NEMS therapy combined with swallowing function training and the control group treated with swallowing function training.Total effective rate and motility of the hyoid laryngeal complex before and after treatment were compared between two groups.Results Through the Putian drinking water test,the total effective rate of the experimental group was higher than that of the control group,and the difference was statistically significant(P<0.05).There was no significant difference in the motility of the hyoid laryngeal complex between the two groups before treatment(P>0.05).The up-and-forward movement of the hyoid bone after treatment in the experimental group were not only greater than that before treatment,but greater than the control group(P<0.05).There were no significant differences in the upward movement and forward movement of the hyoid bone before and after treatment in the control group(P>0.05).There were no significant differences in the up-and-forward thyroid cartilage between the two groups before and after treatment(P>0.05).Conclusion NMES therapy combined with swallowing function training can significantly improve the swallowing function of patients with dysphagia after CAV,which can be applied in clinic.
