摘要
目的:观察钼靶X线和彩色多普勒超声对乳腺癌的影像特征和诊断价值。方法:选取112例乳腺病变患者为研究对象,均接受钼靶X线、多普勒彩色超声检查,观察乳腺癌患者钼靶X线、多普勒彩色超声检查的特征,并将检查结果与病理检查结果这一金标准进行对比,分析其诊断价值。结果:112例乳腺病变患者中,钼靶X线检出乳腺癌68例,表现为结节状肿块影,肿块影多为毛刺状或分叶状,大部分患者有恶性钙化灶,少部分患者有恶性大导管相。多普勒彩色超声检出乳腺癌70例,病变部位多位于上象限,多为不规则形态,边界多不清晰,呈浸润状、毛刺状等,多有内部低回声和微小钙化灶,血流表现为高速、低阻血流,血流强度较高。钼靶X线与多普勒超声诊断效能大致相当,差异无统计学意义(P>0.05);钼靶联合超声诊断灵敏度及准确度较钼靶单独检测更高,差异有统计学意义(P<0.05)。结论:钼靶X线和彩超诊断乳腺癌各有优势,二者联合能够提高乳腺癌诊断准确度、特异度,降低误诊率、漏诊率。
Objective: To observe imaging features and diagnostic values of molybdenum target X-ray and color Doppler ultrasound in breast cancer. Methods: 112 patients with mammary gland lesions were selected as the research subjects, and all of them underwent molybdenum target X-ray and color Doppler ultrasound examinations. The features of molybdenum target X-ray and color Doppler ultrasound examinations were observed, and the results were compared and analyzed with the gold standard of pathological examination results. Then, their diagnostic values were analyzed. Results: For the 112 patients with breast lesions, 68 cases were detected by molybdenum target X-ray, showing nodular masses. Most of them were speculated or lobulated. Most patients had malignant calcifications, and a few patients had malignant large ducts. 70 cases were detected by color Doppler ultrasound. The lesions were mostly located in the upper quadrant and were irregular in the shape. The boundaries were not clear, and were infiltrated and speculated. There were internal low echo and microcalcifications with high speed, low resistance and high signal intensity blood flow. The diagnostic efficiency of molybdenum target X-ray was equivalent to that of Doppler ultrasound, and the difference was not statistically significant(P>0.05). The sensitivity and accuracy of molybdenum target combined with ultrasound were higher than that of single molybdenum target, and the differences were statistically significant(P<0.05). Conclusions: Molybdenum target X-ray and color Doppler ultrasound have their own advantages in the diagnosis of breast cancer. The combination of them can improve the accuracy and specificity of breast cancer diagnosis, and reduce the misdiagnosis rate and missed diagnosis rate.
