血清Gal-3、GDF-15水平与射血分数保留性心力衰竭的相关性研究 被引量:2

摘要 目的:观察射血分数保留性心力衰竭(HFpEF)病人血清半乳糖凝集素3(Gal-3)、生长分化因子15(GDF-15)变化水平,探讨Gal-3、GDF-15对HFpEF的诊断价值及其与心功能分级的相关性。方法:选取2018年10月-2019年10月佳木斯大学附属第一医院心血管病科住院的HFpEF病人60例作为HFpEF组,按照纽约心脏病学会(NYHA)心功能分级标准,将病人分为心功能Ⅱ级组16例、Ⅲ级组24例、Ⅳ级组20例,同时选择30名于我院行体检的健康者为对照组,采用酶联免疫吸附法(ELISA)进行数据检测。结果:与心功能正常组相比,HFpEF组病人血清NT-proBNp、Gal-3、GDF-15水平显著增高,均为p<0.01。Gal-3、GDF-15呈正相关(r=0.66,p<0.05),Gal-3、GDF-15与NT-proBNp水平分别呈正相关(r=0.72,0.65,p<0.05)。结论:血清Gal-3、GDF-15对HFpEF病人的诊断具有重要价值。HFpEF病人血清Gal-3、GDF-15浓度随心功能恶化程度增高而增加,可作为反映心功能损害程度的生物标志物。NT-proBNp联合Gal-3检测或者NT-proBNp联合GDF-15检测,能够提高对HFpEF诊断的准确性。 Objective:To observe the changes of serum galectin-3(Gal-3)and growth differentiation factor 15(GDF-15)in patients with ejection fraction-preserving heart failure(HFpEF),and to explore Gal-3,GDF-15 Diagnostic value of HFpEF and its correlation with cardiac function classification.Methods:From October 2018 to October 2019,60 patients with HFpEF who were hospitalized in the Department of Cardiovascular Diseases of the First Affiliated Hospital of Jiamusi University were selected as the HFpEF group,and the patients were divided into hearts according to the New York Heart Association(NYHA)cardiac function classification standard There were 16 cases in the function group II,24 cases in the class III group,and 20 cases in the class IV group.At the same time,30 healthy people who had undergone physical examination in our hospital were selected as the control group.Results:Compared with the normal heart function group,serum NT-proBNp,Gal-3 and GDF-15 levels in patients with HFpEF group were significantly increased,all of which were p<0.01.Gal-3 and GDF-15 were positively correlated(r=0.66,p<0.05),and Gal-3,GDF-15 and NT-proBNp levels were positively correlated(r=0.72,0.65,p<0.05).Conclusion:Serum Gal-3 and GDF-15 are of great value in the diagnosis of HFpEF patients.The serum levels of Gal-3 and GDF-15 in patients with HFpEF increase with the degree of deterioration of cardiac function,which can be used as a biomarker to reflect the degree of cardiac function impairment.NT-proBNp combined with Gal-3 test or NT-proBNp combined with GDF-15 test can improve the accuracy of HFpEF diagnosis.
