摘要
目的:探讨肺结核大咯血窒息的有效救治方案。方法:回顾性分析24例肺结核大咯血窒息患者的临床资料,判断有创呼吸机辅助通气(IMV)联合选择性支气管动脉栓塞术(BAE)治疗肺结核大咯血窒息的疗效。结果:IMV联合BAE治疗肺结核大咯血窒息的止血有效率为87.50%,其中17例于48 h内成功拔除气管插管,4例于5 d内成功拔除气管插管。结论:利用IMV联合BAE治疗肺结核大咯血窒息,疗效显著,止血率较高,大大提高了抢救成功率。
Objective: To explore the effective treatment plan of pulmonary tuberculosis with massive hemoptysis and asphyxia.Methods: The clinical data of 24 cases of tuberculosis with massive hemoptysis and asphyxia were retrospectively analyzed,and the curative effect of invasive mechanical ventilation(IMV)combined with selective bronchial artery embolization(BAE)was evaluated.Results: The effective rate of hemostasis by IMV combined with BAE was 87.50%.Of 24 cases,17 cases were successfully removed tracheal intubation within 48 hours and 4 cases were successfully removed tracheal intubation within 5 days.Conclusion: IMV combined with BAE in the treatment of pulmonary tuberculosis with hemoptysis and asphyxia has a remarkable effect with a high hemostasis rate,which greatly improves the success rate of rescue.
