摘要
目的:探讨原发性肝癌经导管肝动脉化疗栓塞术(TACE)治疗后CDFI血流信号在患者预后评估中的作用。方法 :收集Ⅱb-Ⅲb期38例原发性肝细胞癌,行TACE治疗后,使用CT、CDFI评估短期疗效,随访患者生存时间、评估长期疗效。结果:CDFI血流信号分级与CT增强扫描表现相关(秩相关检验r=0.38,P=0.019),超声显示肿瘤存在血流信号者,动脉期常强化。随访结果表明,CDFI血流信号分级与生存时间有关联(χ^2=19.36,P<0.000 1),血流信号分级增加,生存时间缩短。结论:CDFI血流信号分级与CT增强扫描表现及生存时间关联。超声在肝癌TACE术后评价中有一定价值。
Objective:To investigate the prognostic role of blood flow signals on ultrasound in patients with primary hepatic carcinoma after interventional therapy with transcatheter arterial chemoembolization(TACE). Methods:The data of 38 patients with hepatocellular carcinoma were collected. After TACE,CT and CDFI were used to examine the tumors of patients to evaluate the short-term therapeutic effect. The survival time of the patients was followed up to evaluate the long-term effect. Results:The blood flow signal grade on CDFI correlated with CT enhancement(rank correlation test r= 0.38,P= 0.019). The tumor blood flow signals detected by CDFI always manifested enhancement on the arterial phase of CT scanning. The blood flow signal grade on CDFI correlated with survival time (χ^2=19.36,P<0.000 1). Patients with more abundant blood flow signals had shorter survival time. Conclusions:Blood flow signal grade on ultrasound is related to CT enhancement and survival time. Ultrasound is of significance in the prognostic evaluation for patients with hepatic carcinoma after TACE treatment.
