维汉患者在腹腔镜食管裂孔疝修补术中丙泊酚的用量比较

Comqarison on the consumption of propofol with laparoscopic esophageal hiatal hernia repair between patients of Uygur and Han nationalities

目的对比分析维吾尔族与汉族食管裂孔疝患者腹腔镜食管裂孔疝修补术中丙泊酚的用量。方法选取2017年1月至2017年12月我院收治食管裂孔疝患者43例作为研究对象,以民族特征分为维吾尔族组(n=22)与汉族组(n=21)。两组诱导和术中维持均采用丙泊酚血浆靶控给药,术中通过调节丙泊酚血浆靶控浓度,维持脑电双频指数(BIS)在45.00±5.00,平均动脉压(MAP)、心率(HR)在基础值±20%的范围内。记录麻醉诱导时(T0)、气腹即刻时(T1)、气腹后1~20 min(T2)、气腹后21~40 min(T3)、气腹后41~60 min(T4)、气腹后61~80 min(T5)、气腹后81~100 min(T6)、气腹后101~120 min(T7)、气腹后121~140 min(T8)、气腹后141 min~气腹停止(T9)各时段丙泊酚用量、CSI、MAP、HR。结果维吾尔族组与汉族组T0丙泊酚用量基本相同,差异无统计学意义(P>0.05);维吾尔族组T1~T9各时段丙泊酚用量以及总用量高于汉族组,差异具有统计学意义(P<0.05)。维吾尔族组与汉族组T1~T9时段CSI、MAP以及HR基本相同,差异无统计学意义(P>0.05)。结论维吾尔族患者丙泊酚的用量明显高于汉族患者,其机制考虑与代谢酶基因遗传多态性有关。

Objective To comqarison of the consumption of propofol with Laparoscopic Esophageal Hiatal Hernia Repair between patients of Uygur and Han nationalities.within scopes of the basic value plus or minus 20%.The consumption of propofol bispectral index(BIS),mean arterial pressure(MAP)and heart rate(HR)values at just at the anesthesia induction(T0),pneumoperitoneum(T1),1~20 min after pneumoperitoneum(T2),21~40 min after pneumoperitoneum(T3),41~60 min after pneumoperitoneum(T4),61~80 min after pneumoperitoneum(T5),81~100 min after pneumoperitoneum(T6),101~120 min after pneumoperitoneum(T7);121~1400 min after pneumoperitoneum(T8),141 min after pneumoperitoneum to stopping pneumoperitoneum(T9)of the two groups were observed and recorded.Results The consumption of propofol at T0 time segments no statistical difference between the two groups(P>0.05);At T1~T9 time segments,the consumption of propofol in the Uygur group were significantly higher than those in the Han group(P<0.05);The MAP and HR at T1~T9 time segments no statistical difference between the two groups(P>0.05).Conclusions The consumption of propofol in Han nationalities is higher to that in Uighurs,which may through Genetic polymorphism of metabolic enzyme gene.
