摘要
目的:探究健康人群与缺铁性贫血(IDA)妇女体内铁蛋白(SF)和红细胞分布宽度(RDW)的表达及其对于IDA的诊断价值。方法:选取160例IDA妇女设为研究组,根据血红蛋白(HGB)含量将研究组分为轻度贫血(HGB>90 g/L)组、中度贫血(HGB 60~90 g/L)组和重度贫血(HGB<60 g/L)组;同时随机选取进行体格检查的50例健康人群为对照组。测定所有研究对象外周血内的SF、脾脏指数(SI)水平以及相关红细胞参数,对两组的结果进行比较并记录。结果:与对照组相比,研究组三组SF下降显著,差异有统计学意义(P<0.05);轻度贫血组SI水平降低不显著(P>0.05);中度贫血组、重度贫血组SI降低显著,差异有统计学意义(P<0.05)。研究组红细胞平均容积(MCV)、平均血红蛋白浓度(MCH)、平均红细胞体积(MCHV)、红细胞分布宽度(RDW)均不同程度降低,差异具有统计学意义(P<0.05)。RDW对于IDA诊断的灵敏度为90.63%,特异度为66%;SF灵敏度为87.5%,特异度为96%。SF联合RDW检测的灵敏度为98.75%,特异度为70%。结论:IDA妇女体内铁蛋白、红细胞分布宽度表达较健康人群降低,铁蛋白、红细胞分布宽度表达降低对于IDA的诊断具有重要价值。SF联合RDW检测能够有效提高IDA检测灵敏度,提高临床早期诊断IDA的能力,对于IDA诊断具有重要价值。
Objective To investigate the expression of ferritin(SF)and red blood cell distribution width(RDW)in healthy people and iron deficiency anemia(IDA)women,and their diagnostic value for IDA.Method A total of 160 IDA women admitted to our hospital were enrolled in the study group.The women in the study group were divided into three groups according to hemoglobin hb content,which were mild anemia(HGB>90g/L).Moderate anemia(HGB 60-90g/L)and severe anemia(HGB<60g/L);at the same time randomly selected 50 healthy people in our hospital for physical examination as a reference group.The levels of SF and SI in the peripheral blood of all subjects and the relevant red blood cell parameters were determined,and the comparison between the two groups was compared and recorded.Results Compared with the reference group,the SF of the ABC group was significantly decreased(P<0.05);the SI level of the group A was not significantly decreased(P>0.05);the SI of the BC group was significantly decreased(P<0.05);the study group MCV,MCH MCHC and RDW were all decreased to different extents,and the difference was statistically significant(P<0.05).The sensitivity of RDW for IDA diagnosis was 90.63%,specificity was 66%;SF sensitivity was 87.5%,specificity was 96%;SF The sensitivity of combined RDW detection was 98.75%,and the specificity was 70%.Conclusion The distribution of ferritin and red blood cell in IDA women is lower than that in healthy people.The decrease of ferritin and red blood cell distribution width is of great value for the diagnosis of IDA.SF combined with RDW detection can effectively improve the sensitivity of IDA detection and improve the ability of early diagnosis of IDA,which is of great value for IDA diagnosis.
