早期预防性护理干预对妊娠期糖尿病患者围生期血糖及妊娠结局的影响

Effect of early preventive nursing intervention on perinatal blood glucose and pregnancy outcome in patients with gestational diabetes

目的:研究早期预防性护理干预对妊娠期糖尿病患者围产期血糖及妊娠结局的影响。方法:选取58例妊娠期糖尿病患者,将其随机分为对照组(n=29)和观察组(n=29)。对照组患者实施常规护理干预,观察组患者实施围产期早期预防性护理干预。比较两组患者的护理满意度,同时比较围产期血糖控制情况以及妊娠结局。结果:护理后,观察组产妇的护理满意度为96.55%,明显高于对照组(72.41%),差异有统计学意义(χ^2=6.44,P=0.01)。在围产期中,观察组产妇的空腹血糖(FBG)、餐后2h血糖(2 h PBG)和糖化血红蛋白(HbA1c)水平均明显低于对照组,差异有统计学意义(P<0.01);观察组孕妇发生胎膜早破、产后出血、剖宫产、羊水过多等不良妊娠结局几率明显低于对照组,差异有统计学意义(P<0.05),而两组患者发生感染的几率比较,差异无统计学意义(P>0.05)。结论:早期预防性护理干预在妊娠期糖尿病患者围产期作用显著,可明显提高孕妇护理满意度,提高对血糖的控制度,促进产妇分娩,改善分娩结局。

Objective To study the effects of early preventive nursing intervention on peripartum blood glucose and pregnancy outcome in patients with gestational diabetes.Method 58 cases of gestational diabetes patients were selected and randomly divided into the control group and the observation group,29 cases in each group.Patients in the control group were treated with routine nursing intervention,and patients in the early preventive nursing intervention group were observed in the perinatal period.The nursing satisfaction of the two groups was compared,and the perinatal blood glucose control and pregnancy outcome were compared.Results After nursing,the nursing satisfaction of the observation group was 96.55%,which was significantly higher than that of the control group(72.41%),the difference was statistically significant(χ^2=6.44,P=0.01).In the perinatal period,the FBG,2 h PBG and HbAlc levels in the observation group were significantly lower than those in the control group,the difference was statistically significant(P<0.01).The incidence of adverse pregnancy outcomes such as premature rupture of membranes,postpartum hemorrhage,cesarean section,and polyhydramnios was significantly lower in the observation group than in the control group,the difference was statistically significant(P<0.05),but there was no significant difference in the incidence of infection between the two groups(P>0.05).Conclusion Early preventive nursing intervention has a significant effect on perinatal period in patients with gestational diabetes,which can significantly improve the nursing satisfaction of pregnant women,improve the control of blood sugar,promote maternal delivery,and improve the outcome of delivery.It is worthy of clinical promotion.
