摘要
目的探讨脑电双频指数(bispectral index,BIS)指导靶控输注丙泊酚在高龄老年人无痛肠镜中的最佳数值范围。方法将2019年1月—2019年6月于本院接受无痛肠镜检查的100例71~80岁的高龄老年人随机分成A组和B组,以脑电双频指数为指导,靶控输注丙泊酚,A组预设初始血浆靶浓度2.0μg,BIS控制在60~70,B组预设初始血浆靶浓度3μg,BIS控制在50~60,进行肠镜检查,记录麻醉诱导时间,麻醉前中后血压,心率,氧饱和度变化,检查时间,清醒时间,丙泊酚用药量,术中知晓,术中体动,术后认知功能,术后不良反应。结果两组在丙泊酚用药量,麻醉中心率,血压,氧饱和度变化,术后认知功能比较有显著差异(P<0.05)。结论脑电双频指数指导丙泊酚靶控输注在高龄老年人无痛肠镜检查中指数范围在60~70效果最佳。
Objective To investigate the optimal numerical range of target-controlled infusion of propofol in elderly patients with painless colonoscopy guided by EEG bispectral index.Methods From January 2019 to June 2019,100 elderly people aged 71 to 80 years old who underwent painless colonoscopy in our hospital were randomly divided into group A and group B.Guided by the electrical double frequency index,target control infusion of propofol,A set of default initial target plasma concentration was 2.0μg,the BIS control in 60~70,the group B default initial concentration of plasma target was 3μg,the BIS control in 50~60,and check endoscopic,record anesthesia induction time,after anesthesia before the blood pressure,heart rate,oxygen saturation,inspection time,waking hours,propofol dose,intraoperative,art,science and technology of postoperative cognitive function,postoperative adverse reaction.Results There were significant differences in propofol dosage,anesthesia center rate,blood pressure,oxygen saturation between the two groups(P<0.05).Conclusion The index range of 60~70 was the best in guiding propofol target-controlled infusion in the painless colonoscopy of the elderly.
