瘢痕子宫再次妊娠分娩方式的选择及安全性临床评价

Clinical Evaluation of Delivery Method and Safety of Re-pregnancy of Scar Uterus

目的探究剖宫产瘢痕子宫妇女进行再次妊娠阴道分娩的效果,并评估其安全性。方法选取2015年8月至2018年1月陆军军医大学士官学校附属医院收治的40例瘢痕子宫再次妊娠产妇并自愿接受阴道分娩方案作为观察组。选取同期收治的40例符合阴道分娩条件的经产妇(第一胎自然分娩)作为对照组,对比两组产妇的产后并发症发生情况。结果对照组阴道分娩成功27例,成功率67.50%;观察组阴道分娩成功26例,成功率65.00%,两组阴道分娩成功率差异无统计学意义(P>0.05)。分娩后,观察组患者的并发症发生率记录为15%,与对照组患者的10%对比无明显统计学差异(P>0.05)。结论只要完全掌握相应适应症,明确禁忌症,剖宫产瘢痕产妇再次妊娠行阴道分娩不会对胎儿和分娩成功率造成影响。

Objective To investigate the effect of re-pregnancy vaginal delivery in women with cesarean scar uterus and to evaluate its safety.Methods From August 2015 to January 201840 pregnant women with scars in the uterus of the hospital were enrolled to receive a vaginal delivery program as an observation group.The 40 women who were eligible for the first birth of the natural childbirth were selected as the control group,and the postpartum complications of the two groups were compared.Results Vaginal delivery was successful in 27 cases in control group(67.50%).The success rate of vaginal delivery in the observation group was 65.00%.There was no significant difference between the two groups(P>0.05).After delivery,the incidence of complications was recorded at 15%in the observation group,and there was no significant difference compared with 10%in the control group(P>0.05).Conclusion As long as the corresponding indications are fully understood and contraindications are clearly defined,vaginal delivery by cesarean section scarred parturient will not affect the fetus and delivery success rate.
