创伤性骨折患者氧化应激水平与止凝血功能的关系探讨

Relationship Between Oxidative Stress Level and Anticoagulation Function in Patients with Traumatic Fracture

目的探讨创伤性骨折患者氧化应激水平与止凝血功能的关系。方法收集2018-2019年沈阳中大骨科医院收治的创伤性骨折患者121例,分为单发骨折组86例与多发骨折组35例;另选择同期健康体检者49例作为对照组。观察各组氧化应激指标[超氧化物岐化酶(SOD)、丙二醛(MDA)]和止凝血功能指标[部分凝血酶时间(APTT)、凝血酶原时间(PT)、纤维蛋白原(FIB)、血小板计数(PLT)、平均血小板容积(MPV)、大血小板比率(P-LCR)]之间的差异。结果三组SOD和MDA水平差异均有统计学意义(P<0.05)。三组APTT、PT、FIB、PLT、MPV和P-LCR水平差异均有统计学意义(P<0.05)。SOD与APTT、PT呈正相关关系(P<0.05);而与FIB、PLT、MPV和P-LCR呈负相关关系(P<0.05)。MDA与APTT、PT呈负相关关系(P<0.05);而与FIB、PLT、MPV和P-LCR呈正相关关系(P<0.05)。结论骨折患者体内存在氧化与抗氧化功能失衡,即氧化应激水平升高,其与骨折病情存在着密切联系。

Objective To investigate the relationship between oxidative stress and anticoagulation function in patients with traumatic fracture.Methods A total of 121 patients with traumatic fracture admitted to our hospital from 2018 to 2019 were divided into single fracture group(86 cases)and multiple fracture group(35 cases).Another 49 healthy people in the same period were selected as control group.The differences of oxidative stress index[superoxide dismutase(SOD),malondialdehyde(MDA)]and anticoagulant function index[partial thrombin time(APTT),prothrombin time(PT),fibrinogen(FIB),platelet count(PLT),mean platelet volume(MPV),platelet-large cell ratio(P-LCR)were observed.Results There were significant differences in SOD and MDA levels among the three groups(P<0.05).There were significant differences in APTT,PT,FIB,PLT,MPV and P-LCR levels among the three groups(P<0.05).SOD was positively correlated with APTT and PT(P<0.05),but negatively correlated with FIB,PLT,MPV and P-LCR(P<0.05).MDA was negatively correlated with APTT and PT(P<0.05),but positively correlated with FIB,PLT,MPV and P-LCR(P<0.05).Conclusion There is an imbalance between oxidation and antioxidant function in patients with fracture,that is,the level of oxidative stress increases,which is closely related to the condition of fracture.
