摘要
整理和归纳林毅教授临床防治乳腺癌复发转移的思路与方法。林毅教授认为乳腺癌复发转移的病机为正气亏虚,余毒未清,痰瘀内阻,毒瘀互结。其中正气亏虚为先决条件,余毒未清是关键因素,痰瘀内阻、毒瘀互结为主要病因。针对其病机的临床治疗原则应为"时时扶正,适时祛邪,随证加减"。扶正固本重在补脾肾,养气血;适时投用祛邪之品需根据机体状况、正邪力量对比来确定;临床诊疗中应根据证候不同进行辨证论治,同时根据其转移部位加减用药,平衡调治。
The thoughts and methods of Professor LIN Yi in preventing and treating breast cancer recurrence and metastasis were introduced. Professor LIN Yi believes that the pathogenesis of breast cancer recurrence and metastasis was characterized by deficiency of healthy qi complicated with residual toxin, internal retention of phlegm and blood stasis, and combination of toxin and blood stasis. Deficiency of healthy qi is the necessary prerequisite to breast cancer recurrence and metastasis,the complication of residual toxin is the key,and internal retention of phlegm and blood stasis plus the combination of toxin and blood stasis is the fundamental element. The therapeutic principle for preventing and treating breast cancer recurrence and metastasis should be supporting healthy qi constantly, eliminating pathogens in time, and modifying the recipe following the syndromes.Supporting healthy qi and strengthening body resistance should be stressed on nourishing spleen and kidney,and invigorating qi and blood,the application of herbs for eliminating pathogens in time should depend on the body condition and the difference between healthy qi and pathogenic qi;and the modification of the recipe should be based on the syndrome differentiation and the location of metastasis,so as to achieve the balanced regulation.
