摘要
目的探讨有氧运动联合肌肉牵张辅助训练对脑卒中偏瘫患者肌力状况、运动功能和生活质量的影响。方法选取2017年10月至2018年6月哈尔滨医科大学附属第一医院神经内科收治的52例脑卒中偏瘫患者为研究对象,采用随机数表法将其分为联合组和对照组,每组各26例。全部患者均给予常规康复治疗和有氧运动干预,在此基础上联合组患者接受肌肉牵张辅助训练,两组均干预4周。比较两组患者干预前后Lovett肌力分级、Fugl-Meyer量表(Fugl-Meyer assessment,FMA)评分和脑卒中专用生存质量量表(stroke-specific quality of life,SS-QOL)评分。结果两组患者干预后Lovett肌力分级较干预前均显著改善(均P<0.05),干预后2周和4周上肢和下肢FMA评分和SS-QOL总分均显著高于本组干预前(均P<0.05)。联合组患者干预后Lovett肌力分级显著优于对照组(P<0.05),干预后2周和4周上肢和下肢FMA评分和SS-QOL总分均显著高于同期对照组(均P<0.05)。结论有氧运动联合肌肉牵张辅助训练对脑卒中偏瘫患者的短期应用效果显著,可增强患者肌力,改善其运动功能和生存质量。
Objective To explore the effect of aerobic exercise combined with assisted muscle stretch training on muscle strength,motor function and quality of life in stroke patients with hemiplegia.Method Fifty-two stroke patients with hemiplegia from October 2017 to June 2018 in the First Affiliated Hospital of Harbin Medical University were enrolled for the study and randomly divided into combined group and control group,26 cases in each group.All patients were given routine rehabilitation treatment and aerobic exercise.On this basis,patients in combined group were given assisted muscle stretch training,and they were all trained for 4 weeks.The Lovett's muscle strength grade,Fugl-Meyer assessment(FMA)score and stroke-specific quality of life(SS-QOL)scale score were compared between the two groups before and after intervention.Result After intervention,Lovett's muscle strength grade was significantly improved in both groups(all P<0.05),and FMA score and SS-QOL total score of upper and lower limbs were significantly higher than those before intervention(all P<0.05).After intervention,Lovett's muscle strength grade in combined group was significantly better than that in control group(P<0.05),FMA score and SS-QOL total score of upper and lower limbs were significantly higher than those of control group at 2 and 4 weeks after intervention(all P<0.05).Conclusion Aerobic exercise combined with assisted muscle stretch training has significant short-term effect on stroke patients with hemiplegia.It can obviously improve the muscle strength,motor function and quality of life of stroke patients with hemiplegia.
