临床药师对337例静脉不合理用药医嘱的干预及效果分析

Clinical pharmacists’ intervention and effect analysis on 337 cases of unreasonable intravenous medication order

目的分析我院临床药师对静脉不合理用药医嘱的审核干预情况,以期提高临床合理用药水平。方法提取我院2019年3~9月静脉用药7573例进行统计分析。结果不合理用药医嘱共计337例,其中干预成功311例,干预成功率为92.28%。不合理医嘱类型前三位的分别是配制溶媒及浓度不合理、过度使用辅助药物、给药时机不合理,分别占20.18%、13.65%、13.35%;干预次数排名前三位的药物分别为注射用头孢尼西钠、注射用环磷腺苷葡胺、丹参川芎嗪注射液,分别占比12.76%、10.39%、10.09%;干预次数排名前三的科室分别为普外科、神经内科和骨科,不合理医嘱分别有45例、42例、40例。结论通过临床药师对不合理医嘱的审核干预,显著提升临床合理用药水平。

Objective To analyze the review and intervention of clinical pharmacists in our hospital on unreasonable intravenous medication order,and to improve the level of clinical rational drug use.Methods 7573 cases of intravenous administration in our hospital from March to September 2019 were collected for statistical analysis.Results There were 337 cases of irrational intravenous administration prescription,including 311 cases of successful intervention,the success rate of intervention was 92.28%.The top three types of irrational intravenous administration prescription were irrational mixed menstruum and concentration,excessive use of auxiliary drugs and irrational timing of administration,accounting for 20.18%,13.65% and 13.35% respectively;cefonicid sodium for injection,meglumine adenosinecyclophosphate for injection and salviae miltiorrhizae and ligustrazine hydrochloride injection ranked the top three intervention frequency,accounting for 12.76%,10.39% and 10.09% respectively;the first three departments for intervention frequency were general surgery department,neurology department and orthopedics department,with 45 cases,42 cases and 40 cases of irrational prescriptions respectively.Conclusion Clinical pharmacist prescription checking intervention is propitious to rational drug use.
