血清脂联素和C反应蛋白的浓度及颈动脉斑块的性质与进展性脑梗死的关系分析 被引量:4

摘要 目的探讨血清脂联素和C反应蛋白的浓度及颈动脉斑块的性质与进展性脑梗死的关系。方法选取2017年2月~2019年2月我院收治的进展性脑梗死患者53例为观察组,另选同时期我院收治的非进展脑梗死者53例为对照组,比较两组患者血清脂联素(APN)、C反应蛋白(CRP)及两组患者检查结果、两组血清抵抗素和瘦素。结果观察组血清脂联素低于对照组,C反应蛋白高于对照组,两者差异均具有统计学意义(P<0.05);观察组颈动脉粥样硬化的发生率、颈动脉斑块数以及中膜厚度,均高于对照组,差异有统计学意义(P<0.05);观察组血清抵抗素和瘦素高于对照组,差异均有统计学意义(P<0.05)。结论C反应蛋白在进展性脑梗死中升高,血清脂联素降低,颈动脉斑块数增加。C反应蛋白是颈动脉不稳定性斑块发生的高危因素,亦是进展性脑梗死发生的高危因素,而血清脂联素则是防止发生进展性脑梗死的保护因素。 Objective To investigate the relationships between serum adiponectin and C-reactive protein concentrations and the properties of carotid plaque with progressive cerebral infarction.Methods Fifty-three patients with progressive cerebral infarction admitted to our hospital from February 2017 to February 2019 were selected as the observation group.Fifty-three patients with non-progressive cerebral infarction admitted to our hospital during the same period were selected as the control group.The serum adiponectin(APN),C-reactive protein(CRP),resistin and leptin as well as examination results of patients in the two groups were compared.Results The serum adiponectin concentration of patients in the observation group was lower than that of patients in the control group,and the C-reactive protein concentration of patients in the observation group was higher than that of patients in the control group,with statistically significant differences(P<0.05).The incidence of carotid atherosclerosis,number of carotid plaque and media thickness of patients in the observation group was higher than those of patients in the control group,with statistically significant differences(P<0.05).The serum resistin and leptin of patients in the observation group were higher than those of patients in the control group,and the difference was statistically significant(P<0.05).Conclusion In progressive cerebral infarction,the C-reactive protein increased,serum adiponectin decreased,and the number of carotid plaques increased.C-reactive protein is a risk factor for the development of unstable carotid plaque and progressive cerebral infarction,and serum adiponectin is a protective factor against the development of progressive cerebral infarction.
