慢性阻塞性肺疾病急性加重期患者肺功能与血气分析、炎症指标的关系分析

Analysis on relationship between lung function and blood gas analysis and inflammatory index in patients with acute exacerbation of chronic obstructive pulmonary disease

目的探讨慢性阻塞性肺疾病急性加重期患者肺功能与血气分析、炎症指标的关系。方法选取2017年1月~2019年1月我院诊断为AECOPD患者70例纳入研究,根据GOLD标准对患者实施分组:轻度组(n=20)、中度组(n=28)、重度组(n=22),对患者均进行肺功能指标(第1秒用力呼气容积/用力肺活量FEV1/FVC、FEV1占预计值百分比FEV1%)检查、血气指标(氧分压PaO2、二氧化碳分压PaCO2、PH值、血氧饱和度SaO2)检查、炎症指标(C反应蛋白CRP、降钙素原PCT、白细胞计数WBC)检查,分析肺功能与血气指标、炎症指标的相关性。结果轻度组、中度组、重度组患者血气指标、炎症指标组间比较,差异有统计学意义(P<0.05);肺功能与PaO2、PH值、SaO2正相关,肺功能与PaCO2、CRP、PCT、WBC负相关,差异有统计学意义(P<0.05)。结论AECOPD患者肺功能与血气分析、炎症指标存在相关性,均可指导疾病诊治过程,临床值得进一步推广。

Objective To explore relationships between lung function and blood gas analysis and inflammatory index in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods 70 patients who were diagnosed as AECOPD from January 2017 to January 2019 in our hospital were selected as research objects.According to the gold standard,they were divided into mild group(n=20),moderate group(n=28),severe group(n=22).Patients were given pulmonary function indexes(forced expiratory volume in 1 second/forced vital capacity FEV1/FVC,FEV1 accounted for the predicted percentage of FEV1%),blood gas index examination(oxygen partial pressure PaO2,carbon dioxide partial pressure PaCO2,PH value,blood oxygen saturation SaO2)and inflammation index examination(C-reactive protein CRP,procalcitonin PCT,white blood cell count WBC).Relationships between lung function and blood gas analysis and inflammatory index were analyzed.Results There were statistically significant differences in blood gas indexes and inflammation indexes between the mild,moderate and severe groups(P<0.05).Lung function was positively correlated with PaO2,PH value and SaO2,and negatively correlated with PaCO2,CRP,PCT and WBC.The difference was statistically significant,(P<0.05).Conclusion Lung function of AECOPD patients is correlated with blood gas analysis and inflammatory indicators,which can guide the diagnosis and treatment of diseases and is worthy of further promotion in clinical practice.
