青春期多囊卵巢综合征患者中医体质与临床指标的相关性分析

Correlation Between TCM constitution and clinical indicators in patients with adolescent PCOS

目的探讨青春期多囊卵巢综合征(PCOS)中医体质分布规律以及不同中医体质与临床指标的相关性,为临床运用辨体、辨病、辨证相结合诊疗模式治疗青春期PCOS提供新的思路。方法选取212例青春期PCOS患者,采用频数分析、Logistic回归分析等统计方法,分析中医体质分布规律及其与临床体征、糖、脂代谢的相关性。结果(1)青春期PCOS患者以气虚质、血瘀质及湿热质多见。(2)青春期PCOS患者中超重占13.68%,肥胖占48.11%;伴痤疮占58.49%,伴溢脂占76.42%。(3)BMI、痤疮、LDL、初潮年龄与气虚质呈正相关,FPG、APOB与气虚质呈负相关;痤疮、FPG、FINS与血瘀质呈正相关,IR、LDL与血瘀质呈负相关;BMI、TC、初潮年龄与湿热质呈正相关,E 2、舒张压与湿热质呈负相关。结论青春期PCOS患者以气虚质、血瘀质、湿热质多见,不同体质与临床指标的相关性存在差异。

Objective To explore the distributive regularity of TCM constitution in adolescent polycystic ovary syndrome(PCOS)and their correlation to provide new ideas for the syndrome differentiation of the body constitution,disease and syndrome.Methods A total of 212 patients with adolescent PCOS were included and frequency analysis,logistic regression analysis were applied to analyze the regularity of constitution and clinical signs,and sugar and lipid indicators.Results(1)The most frequently seen constitutions in adolescent PCOS patients were qi deficiency,blood stasis,and damp heat.(2)The percentage of overweight,obesity,acne and seborrhea in adolescent PCOS patients was 13.68%,48.11%,58.49%,76.42%respectively.(3)BMI,acne,LDL,and menarche age were positively correlated with qi deficiency;FPG and APOB were negatively correlated with qi deficiency.Acne,FPG,and FINS were positively correlated with blood stasis;IR and LDL were negatively correlated with blood stasis.BMI,TC,and menarche age were positively correlated with damp heat;E2 and diastolic pressure were negatively correlated with damp heat.Conclusion In adolescent PCOS patients,qi deficiency,blood stasis,and damp heat are the most common constitutions.There are differences in the correlation between different constitutions and clinical indicators.
