清热化瘀凝胶对Ⅱ型胶原诱导性关节炎大鼠炎症细胞因子表达的影响

Effect of Qingre Huayu Gel on the expression of inflammatory cytokines in rats with type Ⅱ collagen-induced arthritis

目的通过观察清热化瘀凝胶对Ⅱ型胶原诱导性关节炎(collagen-induced arthritis,CIA)大鼠滑膜及血清细胞因子表达的影响,探讨清热化瘀凝胶外敷治疗类风湿关节炎(Rheumatoid Arthritis,RA)的相关作用机制。方法50只SD雌性大鼠随机分为清热化瘀凝胶外敷组、雷公藤多苷片口服组、扶他林外敷组、模型组和正常组,每组10只。观察大鼠关节肿胀度,关节病理切片,肝脏、肾脏组织病理切片,采用ELISA法检测血清及滑膜组织中的肿瘤坏死因子α(TNF-α)、血管内皮生长因子(VEGF)、血管生成素1(Ang-1)和白介素1β(IL-1β)表达差异。结果清热化瘀凝胶外敷组CIA大鼠关节肿胀程度明显减轻;与模型组比较,清热化瘀凝胶组显著降低CIA大鼠血清中IL-1β(P<0.01)、TNF-α(P<0.001)和Ang-1(P<0.01)的水平;滑膜组织中IL-1β(P<0.001)、TNF-α(P<0.01)、VEGF(P<0.01)及Ang-1(P<0.001)较模型组显著下降;关节病理切片显示清热化瘀凝胶可减少炎症细胞浸润、抑制滑膜组织增殖、延缓骨质破坏;肝脏、肾脏病理切片与正常组比较无异常变化。结论清热化瘀凝胶通过降低RA炎症细胞因子水平,抑制滑膜组织增殖,减少滑膜中血管新生和血管翳形成,进而控制RA病情,且安全性良好。

Objective To investigate the effect of Qingre Huayu Gel on synovium and the expression of cytokines in rats with collagen-induced arthritis(CIA)to explore the mechanism of Qingre Huayu Gel in the external treatment of rheumatoid arthritis(RA).Methods Fifty SD female rats were randomly divided into five groups,the Qingre Huayu Gel external application group(TCM group),the tripterygium glycosides oral group(TG group),the Futalin external application group(Futalin group),the model group and the normal group,with10 rats in each group.The joint swelling degree,joint pathological section,liver and kidney pathological sections were observed.Enzyme-linked immunosorbent assay(ELISA)was applied to detect the difference in depression of tumor necrosis factor-α(TNF-α),vascular endothelial growth factor(VEGF),angiogenin 1(Ang-1),and interleukin1β(IL-1β)in serum and synovial tissue.Results Joint swelling in the CIA rats of the TCM group was significantly relieved.Compared with the model group,the TCM group had significant reductions in levels of IL-1β(P<0.01),TNF-α(P<0.001),and Ang-1(P<0.01)in the serum of CIA rats.Compared with the model group,the TCM group had significant reductions in IL-1β(P<0.001),TNF-α(P<0.01),VEGF(P<0.01)and Ang-1(P<0.001)in the synovial tissue.The pathological section showed that the Qingre Huayu Gel could reduce the infiltration of inflammatory cells,inhibit the proliferation of synovial tissue,and delay the bone destruction.Compared with the normal group,there was no difference in the pathological sections of the liver and kidney.Conclusion Qingre Huayu Gel can inhibit the proliferation of synovial tissue,reduce the formation of angiogenesis and vasospasm in synovium,and control the development of RA by reducing the inflammatory cytokine levels.
