摘要
目的探讨初诊2型糖尿病(T2DM)患者血浆Alarin水平及其与糖脂代谢、胰岛素抵抗的相关性。方法选取90例初诊T2DM患者,根据有无肥胖分为T2DM肥胖组40例,T2DM非肥胖组50例,另选取健康体检者30例作为对照组。比较3组血浆Alarin水平,分析T2DM患者Alarin水平与体质指数、糖脂代谢指标及稳态模型胰岛素抵抗(HOMA-IR)指数的相关性。结果对照组、T2DM非肥胖组、T2DM肥胖组的体质指数、三酰甘油、总胆固醇、LDL-C、空腹血糖、糖化血红蛋白A1c(HbA1c)、空腹胰岛素(FINS)、血浆Alarin、HOMA-IR指数依次升高,HDL-L水平依次降低(均P<0.05)。相关分析结果显示Alarin与体质指数、三酰甘油、总胆固醇、LDL-C、空腹血糖、HbA1c、FINS、HOMA-IR指数呈正相关,与HDL-C呈负相关(均P<0.05)。多元线性逐步回归分析结果显示HOMA-IR指数与体质指数均是Alarin水平的影响因素(均P<0.05)。结论初诊T2DM患者血浆Alarin水平升高,其受体质指数和HOMA-IR指数的影响,可能参与T2DM的发生发展过程。
Objective To investigate the plasma Alarin level and its relationship with glycolipid metabolism and insulin resistance in the patients newly diagnosed as type 2 diabetes mellitus(T2DM).Methods Ninety patients newly diagnosed as T2DM were selected,and were divided into T2DM and obesity group(40 cases)and T2DM and non-obesity group(50 cases)according to obesity or non-obesity;then another 30 healthy physical examinees were selected as control group.The plasma Alarin level was compared among the three groups,and the correlation of plasma Alarin level with body mass index,indicators of glycolipid metabolism,and homeostasis model assessment of insulin resistance(HOMA-IR)index was analyzed.Results Body mass index,triglyceride,total cholesterol,LDL-C,fasting blood glucose,glycosylated hemoglobin A1c(HbA1c),fasting insulin(FINS),plasma Alarin,and HOMA-IR index increased,whereas HDL-L level declined in the order of the control group,T2DM and non-obesity group and T2DM and obesity group(all P<0.05).The correlation analysis revealed that Alarin was positively correlated with body mass index,triglyceride,total cholesterol,LDL-C,fasting blood glucose,HbA1c,FINS,and HOMA-IR index,while was negatively correlated with HDL-C(all P<0.05).Multivariate stepwise regression analysis showed that HOMA-IR index and body mass index were the influencing factors for Alarin level(all P<0.05).Conclusion The plasma Alarin level increases in the patients newly diagnosed as T2DM,and is affected by body mass index and HOMA-IR index;besides,it may participate in the occurrence and development of T2DM.
