带状疱疹患者外周血T淋巴细胞亚群的表达及其与病情严重程度的相关性

Expression Levels of T Lymphocyte Subsets in Peripheral Blood and Their Correlations with Disease Severity in Patients with Herpes Zoster

目的探讨带状疱疹患者外周血T淋巴细胞亚群的表达及其与病情严重程度的相关性。方法选取符合纳入及排除标准的带状疱疹100例作为带状疱疹组,健康体检者70例作为健康组,测定两组外周血T淋巴细胞亚群CD3+、CD4+及CD8+水平。比较带状疱疹组和健康组外周血T淋巴细胞亚群CD3+、CD4+及CD8+水平,不同病情程度带状疱疹患者外周血T淋巴细胞亚群CD3+、CD4+及CD8+水平,以及带状疱疹患者治疗前后病情程度评分及外周血T淋巴细胞亚群CD3+、CD4+、CD8+水平变化;采用Spearman相关分析对带状疱疹患者外周血T淋巴细胞亚群CD3+、CD4+、CD8+水平与病情程度相关性进行分析。结果带状疱疹组外周血T淋巴细胞亚群CD3+及CD4+水平低于健康组,CD8+水平高于健康组,差异具有统计学意义(P<0.01)。带状疱疹患者随着病情程度增加,外周血T淋巴细胞亚群CD3+及CD4+水平逐渐降低,CD8+水平逐渐升高,不同病情程度带状疱疹患者总体比较差异有统计学意义(P<0.01)。重度带状疱疹患者外周血T淋巴细胞亚群CD3+、CD4+水平低于轻、中度带状疱疹患者,CD8+水平高于轻、中度带状疱疹患者,差异有统计学意义(P<0.05或P<0.01)。中度带状疱疹患者外周血T淋巴细胞亚群CD3+、CD4+水平低于轻度带状疱疹患者,CD8+水平高于轻度带状疱疹患者,差异有统计学意义(P<0.05或P<0.01)。治疗后,带状疱疹患者病情程度评分及外周血T淋巴细胞亚群CD8+水平与治疗前比较降低,外周血T淋巴细胞亚群CD3+、CD4+水平与治疗前比较升高,差异具有统计学意义(P<0.01)。Spearman相关性分析结果显示,外周血T淋巴细胞亚群CD8+水平与带状疱疹患者病情严重程度呈正相关,外周血T淋巴细胞亚群CD3+、CD4+水平与带状疱疹患者病情严重程度呈负相关。结论带状疱疹患者外周血T淋巴细胞亚群CD3+及CD4+水平低于健康人群,CD8+水平高于健康人群;外周血T淋巴细胞亚群CD3+、CD4+及CD8+水平与带状疱疹患者病情严重程度存在明显相关性。

Objective To explore the expression levels of T lymphocyte subsets in peripheral blood and their correlations with disease severity in patients with herpes zoster. Methods One hundred patients with herpes zoster who met the inclusion and exclusion criteria were selected as the herpes zoster group, and another 70 healthy patients who underwent physical examination were selected as control group(healthy group). The levels of peripheral blood T lymphocyte subsets CD3+, CD4+ and CD8+ were determined and compared in the two groups. The peripheral blood CD3+, CD4+, and CD8+ levels of patients with herpes zoster of different severities, disease severity score of patients with herpes zoster before and after treatment and the changes in peripheral blood T lymphocyte subsets CD3+, CD4+, and CD8+ levels were compared. Spearman correlation analysis was used to explore the correlation between the levels of peripheral blood T lymphocyte subsets CD3+, CD4+ and CD8+ and the severity of disease in patients with herpes zoster. Results The levels of CD3+and CD4+ in peripheral blood T lymphocyte subsets in herpes zoster group were significantly lower than those in healthy group, while the level of CD8+ was significantly higher than that in healthy group(P<0.01). The more severe the disease was, the lower the levels of peripheral blood T lymphocyte subsets CD3+and CD4+ were, and the higher the level of CD8+ was(P<0.01). In patients with severe herpes zoster, levels of T lymphocyte subsets CD3+ and CD4+ were significantly lower than those in patients with mild and moderate herpes zoster, and the levels of CD8+ were significantly higher than those in patients with mild and moderate herpes zoster(P<0.05 or P<0.01). The levels of peripheral blood T lymphocyte subsets CD3+ and CD4+ in patients with moderate herpes zoster were significantly lower than those in patients with mild herpes zoster, and the level of CD8+ was significantly higher than that in patients with mild herpes zoster(P<0.05 or P<0.01). After treatment, the disease severity score and peripheral blood T lymphocyte subset CD8+ levels of patients with herpes zoster were significantly decreased compared with those before treatment, and peripheral blood T lymphocyte subsets CD3+ and CD4+ levels were significantly increased(P<0.01). Spearman correlation analysis showed that the disease severity of patients with herpes zoster was positively correlated with peripheral blood CD8+ level, but was negatively correlated with levels of CD3+and CD4+(P<0.01). Conclusion The expression levels of T lymphocyte subsets CD3+ and CD4+ are significantly lower than those in healthy people, and expression level of CD8+ is higher than that in healthy people. The levels of peripheral blood T lymphocyte subsets CD3+, CD4+, and CD8+ are significantly correlated with disease severity of patients with herpes zoster.
