综合康复治疗对不同时期缺血性脑卒中患者预后的影响

Effect of comprehensive rehabilitation therapy on prognosis of patients with ischemic stroke in different periods

目的探讨综合康复治疗对不同时期脑卒中患者预后的影响。方法回顾性分析2018年1—6月在本科进行康复治疗的缺血性脑卒中患者187例,根据患者发病时间分为3组,急性期86例(发病1个月)、恢复期56例(发病时间2~6个月)、后遗症期45例(发病时间6个月以上),采用综合康复治疗手段包括早期良肢位摆放、体位转移和关节活动度、平衡能力、肌力及行走能力以及日常活动能力等训练后,对三组患者治疗前后进行Brunnstrom、TCT、Barthel指数评分。结果三组患者治疗后Brunnstrom、TCT、Barthel指数评分均较同组治疗前高,差异均有统计学意义(均P<0.05)。结论综合康复治疗对不同时期脑卒中患者的预后均有改善。

Objective To explore the effect of comprehensive rehabilitation therapy on prognosis of patients with ischemic stroke in different periods.Methods 187 patients with ischemic stroke who underwent rehabilitation in our department from January to June 2018 were retrospectively analyzed.According to the patients'onset time,they were divided into 3 groups:86 cases in the acute period(1 month of onset time),56 cases in the recovery period(2 to 6 months of onset time),and 45 cases in the sequela period(more than 6 months of onset time).After application of comprehensive rehabilitation therapy including early good limb positioning,position shift,and training on joint mobility,balance ability,muscle strength,walking ability,and daily activity ability,the three groups were scored of Brunnstrom,trunk control,and Barthel index before and after treatment.Result The scores of Brunnstrom,trunk control,and Barthel index of the three groups after treatment were higher than those before treatment,with statically significant differences(P<0.05).Conclusion Comprehensive rehabilitation treatment can improve the prognosis of patients with ischemic stroke in different periods.
