快速康复在小儿疝囊高位结扎术围手术期护理中的应用效果

Application effect of fast track surgery in perioperative nursing for high ligation of hernial sac in children

目的探讨腹腔镜小儿疝囊高位结扎术围术期快速康复外科理念的应用效果。方法选取本院2018年5月至2019年9月行腹腔镜小儿疝囊高位结扎术的42例患儿为研究对象,数字表法随机分为观察组及对照组,各21例。对照组给予常规护理,观察组在此基础上给予快速康复外科理念的干预护理,观察两组患儿复苏、排气及住院时间,术后舒适度、疼痛程度、护理满意度和并发症发生情况。结果观察组复苏时间[(19.7±9.0)min]、排气时间[(8.6±3.7)h]及住院时间[(1.5±0.27)d]、舒适度[(72.4±16.7)分]、疼痛情况[(1.11±0.14)分]均优于对照组的(32.5±10.3)min、(13.9±4.1)h、(2.6±0.41)d、(50.6±11.3)分和(2.89±0.17)分,两组间比较差异均有统计学意义(均P<0.05);观察组护理满意度明显优于对照组(90.5%比76.2%),两组间比较差异有统计学意义(P<0.05);对照组并发症发生率为14.3%,观察组无并发症发生,但两组间比较差异无统计学意义(P>0.05)。结论腹腔镜小儿疝囊高位结扎术围术期护理应用快速康复外科理念的效果显著,能够提高患儿舒适度和家长满意度,值得推广应用。

Objective To explore the application effect of perioperative fast track surgery concept in laparoscopic high ligation of hernial sac for children.Methods 42 children who underwent laparoscopic high ligation of hernial sac in our hospital from May 2018 to September 2019 were selected as the research objects,and were randomly divided into observation group and control group by the random number table method,with 21 cases in each group.The control group was given routine nursing,and the observation group was given nursing intervention based on the concept of fast track surgery.The recovery,exhaust,and hospitalization time,postoperative comfort,pain degree,nursing satisfaction,and complications of the two groups were observed.Results The recovery,exhaust,and hospitalization time of the observation group were(19.7±9.0)min,(8.6±3.7)h,(1.5±0.27)d,the scores of comfort and pain were(72.4±16.7)points,(1.11±0.14)points,which were significantly better than those of the control group[(32.5±10.3)min,(13.9±4.1)h,(2.6±0.41)d,(50.6±11.3)points,(2.89±0.17)points],with statistically significant differences between the two groups(P<0.05).The nursing satisfaction of the observation group was significantly better than that of the control group(90.5%vs.76.2%),with a statistically significant difference between the two groups(P<0.05).There were no complications in the observation group,the incidence of complications in the control group was 14.3%,but there was no statistically significant difference between the two groups(P>0.05).Conclusion The concept of fast track surgery has significant effects in perioperative nursing of laparoscopic high ligation of hernial sac for children,which can improve the comfort of children and the satisfaction of parents,and is worthy of popularization and application.
