解毒活血化瘀汤对股骨颈骨折术后炎症反应及骨折愈合影响

Effect of Jiedu Huoxue Huayu decoction on postoperative inflammatory response and fracture healing of femoral neck fracture

目的 探讨解毒活血化瘀汤对股骨颈骨折患者术后炎症反应及骨折愈合的影响。方法 选择我院2017年3月~2019年3月收治的股骨颈骨折患者93例为研究对象,采用随机数字表法将其分为对照组46例与观察组47例;对照组采用常规疗法,观察组采用解毒活血化瘀汤治疗,对比两组患者炎症因子、骨折愈合情况、Harris髋关节功能评分及并发症情况。结果 两组患者术后1d IL-6、IL-8、CRP、TNF-α水平比较,差异无统计学意义(P>0.05),治疗结束后观察组各指标均明显低于对照组,差异有统计学意义(P<0.05);观察组骨折愈合率(95.74%)高于对照组(82.61%),差异有统计学意义(P<0.05);两组患者术后1 d Harris评分比较,差异无统计学意义(P>0.05),治疗结束后观察组Harris评分明显高于对照组,差异有统计学意义(P<0.05);观察组骨折不愈合率4.26%,股骨头坏死发生率6.38%,低于对照组骨折不愈合率17.39%,股骨头坏死发生率19.57%,差异均有统计学意义(P<0.05)。结论 股骨颈骨折术后开展解毒活血化瘀汤治疗可有效降低炎症反应,缓解肿胀,促进骨折愈合,提升髋关节功能,值得临床应用与推广。

Objective To investigate the effect of Jiedu Huoxue Huayu decoction on postoperative inflammatory response and fracture healing in patients with femoral neck fracture.Methods A total of 93 patients with femoral neck fractures who were admitted to our hospital from March 2017 to March 2019 were enrolled in the study.They were divided into control group(n=46)and observation group(n=47)according to random number table method.The control group received conventional therapy.The observation group was treated with Jiedu Huoxue Huayu decoction.The inflammatory factors,fracture healing,Harris hip function score and complications were compared between the two groups.Results The levels of IL-6,IL-8,CRP and TNF-αin the two groups were not significantly different at 1 day after treatment(P>0.05).After the treatment,the indexes of the observation group were significantly lower than those of the control group,with statistically significant difference(P<0.05).The fracture healing rate of the observation group was 95.74%,higher than that of the control group(82.61%),and the difference was statistically significant(P<0.05).The difference of Harris score between the two groups at 1 day after treatment was not statistically significant(P>0.05).After treatment,the Harris score of the observation group was significantly higher than that of the control group,and the difference was statistically significant(P<0.05).The fracture nonunion rate was 4.26%in the observation group,and the incidence of femoral head necrosis was 6.38%,which was lower than that of the control group(17.39%and 19.57%).The difference was statistically significant(P<0.05).Conclusion The treatment of Jiedu Huoxue Huayu decoction after femoral neck fracture can effectively reduce inflammation,relieve swelling,promote fracture healing and improve hip function.It is worthy of clinical application and promotion.
