新生儿黄疸与凝血酶原时间、活化部分凝血酶原时间延长的相关性及临床价值 被引量:3

Relationship Between Neonatal Jaundice and Prolongation of Prothrombin Time and Activated Partial Thromboplastin Time and Its Clinical Value

摘要 目的:探究新生儿黄疸与凝血酶原时间(PT)、活化部分凝血酶原时间(APTT)延长的相关性及临床价值。方法:选取2017年1-12月在本院进行新生儿黄疸治疗的70例患儿作为研究对象,依据病理类型分为病理性黄疸组与生理性黄疸组,各35例,选取同一时期的35例健康新生儿作为对照组。通过对三组新生儿的CRP水平、总胆红素水平以及凝血功能指标(PT、APTT、TT、FIB)进行检测,从而对检测结果进行分析比较。结果:病理性黄疸组与生理性黄疸组CRP与总胆红素水平均明显高于对照组,而病理性黄疸组CRP、总胆红素水平均显著高于生理性黄疸组(P<0.05);病理性黄疸组PT、APTT、TT指标均明显高于生理性黄疸组与对照组(P<0.05),FIB水平均低于生理性黄疸组与对照组(P<0.05);随着胆红素水平升高,患儿PT、APTT均显著延长(P<0.05)。结论:新生儿黄疸能够导致其PT、APTT显著延长,对新生儿实行CRP以及总胆红素水平的检测,能够及时发现新生儿病理性黄疸的存在情况,进而便于治疗方案的确立以及有效实施。 Objective: To explore the correlation and clinical value of neonatal jaundice with prothrombin time(PT) and prolongation of activated partial thromboplastin time(APTT). Method: A total of 70 cases of neonatal jaundice treated in our hospital from January to December 2017 were selected as the research objects. According to the pathological type, they were divided into pathological jaundice group and physiological jaundice group, 35 cases in each group, 35 healthy newborns in the same period were selected as the control group. The CRP level, total bilirubin level and coagulation function indexes(PT, APTT, TT, FIB) of three groups of newborns were detected, and the results were analyzed and compared. Result: The levels of CRP and total bilirubin in the pathological jaundice group and the physiological jaundice group were significantly higher than those in the control group, while the CRP and total bilirubin levels in the pathological jaundice group were significantly higher than those in the physiological jaundice group(P<0.05). The PT, APTT and TT indexes of the pathological jaundice group were significantly higher than those of the physiological jaundice group and the control group(P<0.05), the FIB levels were lower than those of the physiological jaundice group and the control group(P<0.05). With the increase of bilirubin level, PT and APTT were significantly prolonged(P<0.05). Conclusion: Neonatal jaundice can lead to significant prolongation of PT and APTT. The detection of CRP and total bilirubin level can detect the existence of neonatal pathological jaundice in time, which is convenient for the establishment and effective implementation of treatment plan.
