快速康复外科理念在腹腔镜下全子宫切除患者中的应用

Application of Rapid Rehabilitation Surgery Concept in Patients Undergoing Laparoscopic Total Hysterectomy

目的:探讨快速康复外科理念在腹腔镜下全子宫切除术中的应用价值。方法:选取2017年1月-2019年1月本院收治的行腹腔镜下全子宫切除术患者60例,按随机数字表法分为对照组与观察组,各30例。对照组采取常规护理措施,观察组在对照组的基础上采取快速康复外科理念。比较两组术后首次排气、首次进食及住院时间,比较两组并发症、生活质量及护理满意情况。结果:观察组术后首次排气时间(14.5±3.1)h、首次进食时间(6.3±0.7)h、住院时间(3.7±1.2)d均低于对照组的(23.3±3.2)h、(9.4±2.7)h、(6.9±1.3)d,差异均有统计学意义(t=10.818、6.087、9.907,P=0.000)。观察组并发症发生率23.33%低于对照组的70.00%(X^2=13.125,P<0.001)。观察组各项生活质量评分均高于对照组(P<0.05)。观察组护理总满意率为96.7%高于对照组的80.0%(X^2=4.043,P=0.044)。结论:快速康复外科理念在腹腔镜下全子宫切除术中的应用效果显著,可减少患者相关并发症,促进术后快速恢复,提升生活质量和满意度,值得临床推广和应用。

Objective: To explore the application value of the rapid rehabilitation surgery concept in laparoscopic total hysterectomy. Method: From January 2017 to January 2019, 60 patients underwent laparoscopic total hysterectomy in our hospital were selected and divided into the control group and the observation group according to the random number table method, 30 cases in each group. The control group was given routine nursing, and the observation group was given the rapid rehabilitation surgery concept on the basis of the control group. The first postoperative exhaust, feeding and hospitalization time were compared between the two groups. Complications, quality of life and nursing satisfaction were compared between the two groups. Result: The first postoperative exhaust time(14.5±3.1) h, first feeding time(6.3±0.7) h, and hospitalization time(3.7±1.2) d in the observation group were all lower than(23.3±3.2) h,(9.4±2.7) h,(6.9±1.3) d in the control group, the differences were statistically significant(t=10.818, 6.087, 9.907, P=0.000). The incidence of complications in the observation group was 23.33% lower than 70.00% in the control group( X^2=13.125, P<0.001). The all quality of life scores in the observation group were higher than those in the control group(P<0.05). The total satisfaction rate of nursing in the observation group was 96.7% higher than 80.0% in the control group( X^2=4.043, P=0.044). Conclusion: The application of rapid rehabilitation surgery concept in laparoscopic total hysterectomy has a significant effect, which can reduce the related complications, promote rapid postoperative recovery, improve the quality of life and satisfaction of patients. It is worthy of clinical promotion and application.
