COOK双球囊与米索前列醇在低宫颈评分初产足月妊娠孕妇引产中的应用比较

Comparison of the Applications of COOK Double Balloon and Misoprostol in Induction of Labor in Primiparous Full-Term Pregnant Women with Low Cervical Score

目的:比较COOK双球囊与米索前列醇在低宫颈评分初产足月妊娠孕妇引产中的应用。方法:选取本院收治的120例低宫颈评分初产足月妊娠拟引产的孕妇。按治疗方法的不同分为对照组(n=59)与研究组(n=61),对照组给予小剂量米索前列醇片治疗,研究组给予COOK双球囊治疗。比较两组的宫颈Bishop评分、促宫颈成熟效果、临产发动时间、剖宫产率、阴道分娩率、产后出血发生率及宫颈裂伤率。比较两组的阴道分娩产妇的阴道助产率、总产程时间及阴道分娩出血量。结果:用药后,研究组宫颈Bishop评分明显高于对照组(P<0.05),且研究组促宫颈成熟总有效率为96.72%,明显高于对照组的72.88%(P<0.05)。研究组阴道分娩率明显高于对照组(P<0.05),研究组临产发动时间及剖宫产率均低于对照组(P<0.05)。研究组阴道分娩孕妇总产程时间及阴道分娩出血量均明显低于对照组(P<0.05);研究组产后出血率及宫颈裂伤率均明显低于对照组(P<0.05)。结论:在低宫颈评分初产足月妊娠孕妇引产中应用COOK双球囊可减少剖宫产率,更具安全性、有效性与稳定性,具有较高的应用价值。

Objective:To compare and the application of COOK double balloon and Misoprostol in induction of labor in primiparous full-term pregnant women with low cervical score.Method:A total of 120 pregnant women with low cervical score admitted to our hospital plan to induce labor were selected.According to the different treatment methods,they were divided into the control group(n=59)and the study group(n=61).The control group was treated with low dose Misoprostol Tablets,while the study group was treated with COOK double balloon therapy.The score of cervical Bishop,the effect of cervical maturation,the time to start labor,the rate of cesarean section,the rate of vaginal delivery,the incidence of postpartum hemorrhage and the rate of cervical laceration of the two groups were compared.The vaginal midwifery rate,total labor duration and vaginal delivery bleeding volume were compared between the vaginal delivery parturient in two groups.Result:After medication,the score of cervical Bishop in the study group was significantly higher than that in the control group(P<0.05).And the total effective rate of cervical maturation in the study group was 96.72% significantly higher than 72.88% in the control group(P<0.05).The rate of vaginal delivery in the study group was significantly higher than that in the control group(P<0.05).The time to start labor and cesarean section rate in the study group were lower than those in the control group(P<0.05).The total duration of labor and the amount of bleeding during vaginal delivery of vaginal delivery parturient in the study group were significantly lower than those in the control group(P<0.05).The rate of postpartum hemorrhage and cervical laceration in the study group were significantly lower than those in the control group(P<0.05).Conclusion:COOK double balloon can reduce cesarean section rate in induction of labor in primiparous full-term pregnant women with low cervical score,which is more safe,effective and stable,and has higher application value.
